REGISTRATION AGREEMENT, WAIVER & RELEASE OF LIABILITY
By selecting "I Agree" during registration, I acknowledge that I have read, understood, and agree to be bound by the terms outlined below on behalf of myself and/or the participant(s) I am registering.
ACKNOWLEDGEMENT OF POLICIES
I acknowledge that I have reviewed the current Thrive Dance Studio Information Package, Competitive Information Package (if applicable), and all published studio policies. I understand that these documents form part of my registration agreement with Thrive Dance Studio Inc. and Thrive Dance Society.
I understand that it is my responsibility to remain informed of studio policies, procedures, schedules, and communications throughout the season.
COMMUNICATION
I acknowledge that email is Thrive Dance Studio's primary method of communication and agree to maintain current contact information with the studio. I understand that important information regarding classes, schedules, performances, fees, and studio operations will be communicated electronically.
REGISTRATION, TUITION & FEES
I understand and agree that:
• Registration Fee, Society Membership Fee, and Media Fee are non-refundable.
• Tuition and program fees are due according to the payment schedule established by Thrive Dance Studio.
• Tuition is based on annual enrollment and is not adjusted for holidays, absences, illness, vacations, school closures, or varying numbers of classes per month.
• No refunds, credits, or make-up classes will be provided for missed classes unless otherwise stated by the studio.
• Additional fees may apply, including but not limited to costumes, performances, examinations, competitions, workshops, camps, private lessons, specialty programs, and facility-related expenses.
•The Ballet Production Fee is non-refundable upon payment.
• Musical Theatre Production Fees, Acro/Silks Production Fees, costume deposits, costume fees, and any other performance-related deposits become non-refundable after January 1, 2027.
• Withdrawal from a program does not entitle the participant to a refund of any non-refundable fees or deposits already paid.
• Any outstanding balances remain the responsibility of the account holder.
• NSF payments, declined transactions, chargebacks, or returned payments may be subject to administrative fees.
I authorize Thrive Dance Studio Inc. to process payments using the payment method provided during registration for all fees owing under this Agreement.
THRIVE DANCE SOCIETY MEMBERSHIP
Where applicable, I understand that participation in Thrive Dance Studio programs includes membership in Thrive Dance Society and any associated membership obligations, volunteer commitments, and fees as outlined during registration.
I authorize Thrive Dance Society to process any applicable membership fees, media fees, volunteer buyout fees, or other approved society charges.
PARTICIPATION EXPECTATIONS
I understand that students are expected to attend classes regularly, arrive on time, and comply with studio expectations regarding conduct, dress code, safety, and respectful behaviour.
Thrive Dance Studio reserves the right to suspend or remove any participant whose conduct is disruptive, unsafe, inappropriate, or contrary to the values and expectations of the studio.
COMPETITIVE PROGRAM PARTICIPATION
For participants enrolled in competitive programs, I acknowledge that participation represents a season-long commitment and that additional policies, expectations, fees, and attendance requirements apply.
I understand that competition registrations, teacher fees, choreography fees, costume fees, and other competition-related expenses may become non-refundable once commitments have been made on the participant's behalf.
I further acknowledge that withdrawals from competitive groups may result in additional restructuring or rechoreography fees as outlined in the Competitive Information Package.
PHOTOGRAPHY, VIDEO & MEDIA RELEASE
I grant Thrive Dance Studio Inc., Thrive Dance Society, and their authorized representatives permission to photograph, film, record, livestream, or otherwise capture the participant's image, voice, likeness, performances, and participation in studio activities.
I understand that these materials may be used for promotional, educational, archival, marketing, social media, website, broadcast, or operational purposes without compensation.
I acknowledge that all photographs, recordings, and media produced by or on behalf of Thrive Dance Studio remain the property of Thrive Dance Studio Inc. and/or Thrive Dance Society.
ASSUMPTION OF RISK
I acknowledge that participation in dance classes, fitness classes, aerial arts, acrobatics, conditioning programs, workshops, camps, performances, competitions, rehearsals, drop-in classes, specialty programs, and all other activities offered by Thrive Dance Studio involves inherent risks.
These risks may include, but are not limited to:
• Slips, trips, falls, and collisions;
• Physical exertion and fatigue;
• Equipment malfunction or failure;
• Sprains, strains, fractures, concussions, and other injuries;
• Permanent disability;
• Serious injury or death.
I voluntarily accept and assume all risks associated with participation in studio activities.
I further represent that the participant is physically capable of participating in the activities for which they are registered and that any relevant medical conditions, allergies, injuries, or limitations have been disclosed to the studio.
RELEASE OF LIABILITY
To the fullest extent permitted by law, I release, waive, discharge, and hold harmless Thrive Dance Studio Inc., Thrive Dance Society, their directors, officers, owners, employees, contractors, volunteers, instructors, agents, representatives, successors, and assigns from any and all claims, demands, causes of action, liabilities, damages, costs, expenses, or losses arising out of or relating to participation in any studio activity.
This release includes, without limitation, claims arising from personal injury, illness, property damage, loss, disability, or death resulting from participation in studio activities, except where prohibited by applicable law.
EMERGENCY MEDICAL AUTHORIZATION
In the event of an accident, illness, injury, or emergency involving the participant, I authorize Thrive Dance Studio personnel to seek emergency medical treatment when reasonable efforts to contact a parent, guardian, emergency contact, or participant cannot be made in a timely manner.
I understand that any medical, dental, ambulance, transportation, or related expenses incurred remain my responsibility.
I release Thrive Dance Studio Inc., Thrive Dance Society, and their representatives from liability arising from actions reasonably taken in response to a medical emergency.
STUDIO OPERATIONS
I understand that class schedules, instructors, programming, performance opportunities, studio facilities, and services may be modified, substituted, postponed, relocated, or cancelled at the discretion of Thrive Dance Studio.
In the event of temporary facility closures, emergencies, public health concerns, or circumstances beyond the studio's control, Thrive Dance Studio may provide alternative programming, including virtual instruction or adjusted schedules.
ELECTRONIC CONSENT
By selecting "I Agree" during registration, I acknowledge that this electronic acceptance constitutes my legal signature and confirms my agreement to all terms and conditions contained within this Registration Agreement, Waiver, and Release of Liability.
I certify that I am at least eighteen (18) years of age or am the parent/legal guardian of the participant being registered and have the authority to enter into this Agreement.
