Agreement to Policies, Guidelines, and Liability Release
I hereby agree to abide by all Studio 8 Performing Arts Policies and Guidelines while I and/or my child are enrolled in the dance program, effective as of the date of this agreement. I understand that it is my responsibility to read and follow all rules and regulations outlined in the Policies and Guidelines, which will be made available through the Welcome Packet located in this portal, the Parent Portal (Files section) on Facebook, the Studio 8 Performing Arts BAND parent group page, and the official Studio 8 Performing Arts website.
By registering myself and/or my child with Studio 8 Performing Arts, I further agree to adhere to all insurance policies established by the studio. I acknowledge and agree that Studio 8 Performing Arts, its owners, staff, and affiliates shall not be held liable for any accidents, injuries, or losses that may occur while participating in studio activities, whether on or off the premises. I hereby release Studio 8 Performing Arts from any and all claims or liabilities arising from participation in classes, rehearsals, performances, or related events.
