Parent Video Monitoring & Spot TV Waiver

I understand and acknowledge that DanzForce Extreme utilizes cameras and video monitoring within its facility through Spot TV for purposes that may include class observation, training, management, safety, and studio operations. My child(ren) may appear in livestreamed or recorded video while participating in classes, rehearsals, or other activities at DanzForce Extreme.

By accepting this waiver, I give permission for myself and/or my minor child(ren) to be photographed, video recorded, and/or audio recorded while participating in activities at DanzForce Extreme.

I understand that authorized parents/guardians may be provided secure access to view certain video content through Spot TV. Because other children, instructors, staff members, and individuals may appear in this content, I agree to respect their privacy.

I understand and agree that I will not screenshot, screen-record, download, copy, reproduce, share, distribute, publish, alter, or otherwise capture or use any livestream, video, photograph, audio recording, or other content accessed through Spot TV.

I will not share my Spot TV login credentials or provide unauthorized individuals with access to the service. I understand that I am responsible for the use of my account and for ensuring that members of my household who may have access comply with these requirements.

I understand that unauthorized recording, copying, sharing, reproduction, or distribution of Spot TV or DanzForce Extreme content may violate copyright, privacy, or other applicable laws and may result in the suspension or termination of my Spot TV access and/or other action deemed appropriate by DanzForce Extreme or Spot TV.

I understand that Spot TV is a third-party service and that use of its platform may also be subject to Spot TV's current Terms of Service and Privacy Policy.

Electronic Acknowledgment & Consent

By clicking “I Agree” below, I acknowledge that I am the parent or legal guardian of the child(ren) being registered, that I have read and understand this Parent Video Monitoring & Spot TV Waiver, and that I voluntarily consent to and agree to its terms on behalf of myself and my minor child(ren). I understand that my electronic acceptance of this waiver is intended to serve as my acknowledgment and agreement to these terms.
