Woodland Dance Center (WDC) Participation Waiver and Agreement

Acknowledgement of Risk

I, the undersigned (student/guardian/parent), recognize and acknowledge that participation in activities at Woodland Dance Center (WDC) involves inherent risks of physical injury. I voluntarily assume full responsibility for any injuries, damages, or losses that I or my child may sustain as a result of participating in any and all WDC activities.

Medical Authorization

In the event of an emergency, I authorize WDC teachers or staff to secure from any licensed hospital, physician, and/or trained medical personnel any treatment deemed necessary for the immediate care of my child. I agree that I, as parent/guardian/student, am financially responsible for any and all medical services rendered.

Injury Disclosure and Participation Restrictions

I understand that it is my responsibility to notify WDC staff of any known injuries, illnesses, or physical limitations that may affect my child’s (or my own) participation in class. WDC reserves the right to restrict or deny participation to any student who, in the judgment of staff, may risk further injury by dancing. Students with injuries may be asked to sit out, observe, or participate in modified activities as deemed appropriate by WDC staff.

Attendance and Make-Up Policy

I understand that WDC follows the Woodland School District calendar for holidays and spring break. No regular dance classes will be held during these breaks unless otherwise specified, and there will be no make-up classes for these scheduled closures. If I or my child misses a class, I acknowledge that WDC and its instructors are under no obligation to provide a make-up class, private lesson, or reimbursement.

I also understand that consistent attendance is essential for class progress. Multiple unexcused absences may result in removal from class without refund.

Class Expectations

I agree that my child will:

- Arrive on time for all classes.

- Be dressed according to the WDC dress code (e.g., ballet: black or pink leotard, pink tights, ballet shoes, hair pulled back off the neck).

- Understand that if more than 15 minutes late, the student will not participate physically but may take notes and observe.

Tuition and Payment Policy

I agree to pay full tuition on the first day of each month (September 2025 – June 2026), or beginning at the time of enrollment for mid-year students. If payment is not received by the 5th of the month, I agree to pay a $10 late fee per student. I understand that monthly tuition and registration fees are non-refundable. A minimum of 20 days’ notice is required for withdrawal from class.

Media Release

I understand that photographs and/or videos may be taken during WDC classes, rehearsals, and performances. These may be used for publicity, promotional materials, social media, or WDC displays. If I wish to opt out of having my child’s image used, I will provide a signed, written request to the WDC Board of Directors.

Governing Law and Venue

This agreement shall be governed by and construed under the laws of the State of Washington. Any disputes arising under this agreement shall be brought exclusively in the courts of Cowlitz County, Washington.

Release of Liability

By signing this waiver, I release and hold harmless Woodland Dance Center, its owners, instructors, employees, and agents from any and all claims, liabilities, demands, or causes of action arising out of or related to participation in WDC activities, except in cases of gross negligence or willful misconduct as defined under Washington law.

Indemnification

In addition, I agree to indemnify, defend, and hold harmless Woodland Dance Center, its owners, instructors, employees, and agents from and against any and all claims, demands, damages, costs, expenses, or liabilities (including attorney’s fees) arising out of or related to my child’s participation in WDC activities, including claims brought by other participants, parents, or third parties.
