Talbot Dance Academy - Concert Consent Form 2025
By agreeing to this waiver you are consenting to your child participating in the TDA 2025 Concert as well as the following:
*You are agreeing to pay the compulsory concert fee of $25. The $25 fee concert fee covers your child's attendance in the concert and goes towards filming, music rights, lighting costs, college hall fees and staff costs.
*You are agreeing to participate in the 2025 Concert
*You agree to settle any remaining unpaid accounts by Mon 10th Nov Any families with unpaid accounts will unfortunately not be able to participate in our 2025 Concert.
*You consent for your son/ daughter to be filmed and photographed as part of Talbot Dance Academy's 2025 end of year concert. Video footage will be available to those students who have performed and photographs will be available for Talbot Dance Academy students/ families to purchase. Talbot Dance Academy may also use the video footage and photographs for promotional purposes.
