The Dance Studio – Agreement & Waiver
1. Liability Release
-I understand and agree that Angela Vital-Martowski, D/B/A The Dance Studio of Wilbraham, its agents, employees, and representatives are not responsible for any injuries sustained while participating in classes, rehearsals, performances, or events — on or off the premises.
-For the purposes of this agreement, “premises” refers only to the specific areas included in The Dance Studio’s rental agreement, not the building or property as a whole.
2. Privacy / Media Release
-I consent to being audio recorded, video recorded, and/or photographed during classes, rehearsals, and performances.
-Recordings may be used by The Dance Studio only, including for websites, social media, and promotional materials.
-I understand and agree that by entering the studio, I may be video and audio recorded for security and safety purposes by security cameras owned and operated by The Dance Studio.
-No photos, videos, or audio recordings will be posted on personal social media accounts belonging to staff without my consent.
3. Fall–Spring Tuition & Fees
-I agree to pay all tuition, costume, and/or performance fees to The Dance Studio.
-If I wish to drop a class or activity, I will notify the studio before the first day of the next payment period. If not, I am responsible for that period’s tuition and any related fees in full.
-All payments are due the first week of each month.
-Accounts over 30 days past due will be charged a $20 late fee.
-If my account is 60 days past due, the dancer will not be permitted to return to class until the balance is paid in full.
-Payments can be made by cash, check, or credit card through the studio pro parent portal.
4. Summer Tuition & Fees
-I agree to pay all fees due for summer programs at The Dance Studio.
-I understand that deposits are non-refundable and non-transferable and secure my spot in the activity.
-Remaining balances for summer camps/workshops/intensives will be paid by cash or check directly to the teacher on the first day of the program.
-Tuition for summer evening classes and mini camps is due at registration.
-Registration is not complete until payment is received in full.
-I will notify the studio at least 7 days prior to dropping a summer class or activity.
