Recurring Payment Agreement
By signing below, you consent to the terms of this Agreement as set forth herein:
• You authorize Tapping with Tina’s Ballerinas, to store in our secure records the credit card identified with your tuition payment information. You also understand that you are required to update that credit card information whenever necessary.
• You give permission to Tapping with Tina’s Ballerinas to charge the credit card on file the monthly class fees plus a 4% convenience fee on the 1st week of each month your dancer(s) is enrolled, which will start October 2026 and continue until May 2027. You authorize Tapping with Tina’s Ballerinas to initiate a recurring charge for this tuition each month during this time period.
•. You understand that you may use the credit card on file to charge incidental and extra charges, such as costume payment or recital fees, but that you are not required to charge these expenses. However, you authorize Tapping with Tina’s Ballerinas to charge the card on file any such expenses remain unpaid within 24 hours of the due date.
• You agree that this authorization is continuous and will automatically apply to any future sessions, unless Tapping with Tina’s Ballerinas is instructed differently.
• In the event you are owed a credit, Tapping with Tina’s Ballerinas will provide you with notice and will refund the applicable amount to the card on file.
• This Agreement will expire when you either no longer have a child participating with Tapping with Tina’s Ballerinas or when full payment of your balance has been received by Tapping with Tina’s Ballerinas for the session. For clarity, if you have an outstanding balance with Tapping with Tina’s Ballerinas, this Agreement will remain in effect until your balance is paid whether or not your dancer continues dancing.
• You understand that it is your responsibility to have the necessary funds available to cover the amount of the monthly payments due to Tapping with Tina’s Ballerinas. If you do not, an automatic $10 late fee will be applied to your balance. Tapping with Tina’s Ballerinas will NOT be responsible for any bank fees incurred due to the policies of your banking institution or insufficient funds in your account. However, in the event that Tapping with Tina’s Ballerinas incurs fees due to insufficient funds in your account, you will be responsible for those fees.
• By signing below, you guarantee that you authorized to provide this account as a payment account for your tuition and fees owed to Tapping with Tina’s Ballerinas. You also agree to keep all credit card information updated and notify Tapping with Tina’s Ballerinas of any changes no less than 21 days prior to the next scheduled withdrawal date.
• Tapping with Tina’s Ballerinas may make changes to this Agreement by providing you with 30 days prior written notice. You may cancel this agreement by giving notice of cancellation in writing to Tapping with Tina’s Ballerinas 30 days prior to cancellation.
• If you make changes to, or are asked to update, your payment method or information, you may be required to consent to a revised or updated Recurring Payment Agreement or similar agreement with Tapping with Tina’s Ballerinas at that time.
Tapping with Tina’s Ballerinas will not complete any recurring transaction after receiving: (i) a cancellation notice from the Cardholder, (ii) notice from EVO or Bank, or (iii) a response that the Card is not to be honored.
