2026-2027 Participation Waiver

Liability Release:
I hereby waive, release and hold harmless Dance Dimensions of Hedgesville, LLC, its owner, and instructors from any and all liability or claims resulting from any injury to me or my child due to our participation in any class/program offered at and/or by Dance Dimensions of Hedgesville, LLC. I understand that tuition payments are non-refundable and non-transferable. Furthermore, I agree to not deduct missed classes from payments. I am aware that the child above must be picked up on time or I will be held responsible for a late dismissal fee (as outlined in the 2026-2027 Dance Dimensions Studio Polices Handbook).
Photo/Media Release:
I, or as legal guardian of this child under the age of 18, give permission for Dance Dimensions of Hedgesville, LLC to use the enrolled participants photograph in all forms and media, when representing the 2026-2027 dance year whether electronic, print, digital, or electronic publishing via the Internet for advertising and any other lawful purpose.
2027 Recital:
Should my child opt in to participate in the 2027 recital, I understand that my account must balance must be paid in full for my child to participate and I give permission for Dance Dimensions of Hedgesville, LLC to print on the enrolled participant's name on the 2027 recital t-shirt and program.
