LIABILITY WAIVER & RELEASE OF CLAIMS

I, the undersigned parent/guardian, understand that participation in dance classes, rehearsals, performances, competitions, workshops, and other activities provided or sponsored by Momentum Dance Center involves physical activity and may involve certain risks, including but not limited to falls, slips, collisions, muscle strains, sprains, fractures, or other injuries.

I understand that dance requires physical exertion, stretching, jumping, turning, lifting, balancing, and other movements that may result in injury, even when appropriate instruction, supervision, equipment, and safety precautions are provided.

By signing this waiver, I acknowledge and voluntarily assume the risks associated with participation in Momentum Dance Center activities.

RELEASE OF LIABILITY

To the fullest extent permitted by law, I agree to release and hold harmless Momentum Dance Center, its owners, instructors, employees, independent contractors, volunteers, and representatives from claims, demands, damages, liabilities, or expenses arising from or related to my child’s participation in Momentum Dance Center activities, including claims relating to personal injury, illness, property damage, or loss, except to the extent caused by conduct that cannot legally be waived.

I understand that Momentum Dance Center and its instructors will take reasonable precautions to provide a safe environment; however, injuries and accidents can occur despite appropriate safety measures.

MEDICAL TREATMENT

In the event of an injury or medical emergency involving my child, I authorize Momentum Dance Center staff to take reasonable steps to obtain emergency medical care when I cannot be reached or when immediate action is reasonably necessary.

I understand that Momentum Dance Center is not responsible for medical expenses, ambulance transportation, emergency treatment, or other costs resulting from an injury or illness.

RESPONSIBILITY OF PARENT/GUARDIAN

I agree to inform Momentum Dance Center of any medical condition, injury, physical limitation, allergy, or other circumstance that may affect my child’s ability to safely participate in dance activities.

I understand that my child is expected to follow instructor directions, studio rules, and safety guidelines. I understand that failure to follow these rules may increase the risk of injury.

I acknowledge that participation in dance is voluntary and that I have had the opportunity to ask questions regarding the activities and associated risks.

ACKNOWLEDGMENT

By signing below, I confirm that I have read and understand this Liability Waiver and Release of Claims. I understand that I am voluntarily allowing my child to participate in Momentum Dance Center activities and that I am accepting the risks associated with participation.
