WAIVERS AND RELEASE OF LIABILITY

Disclaimer: Dancing Stars Dane Studio is not responsible for any injury, or loss of property to any person while practicing, taking class, participating in class, birthday parties, or any other way involved in the Dancing Stars facility. Dancing Stars Dance Studio for any reason whatsoever, including ordinary negligence on the part if Dancing Stars Dance Studio, its owners, employees, and independent contractors.
I am aware that these classes involve physical activates that involve body movement, rotations, and intense stretching and as such they pose a risk for injury. The risk may be limited due to the trained staff but never eliminated. I understand that such activities always involve certain risk, including but not limited to; injury to virtually all bones, joints, muscles, and internal organs. I affirm that I am legal age and freely signing this agreement. I have read this form fully and understand that by signing this form I am giving up legal rights and/or remedies which may be available to me for the ordinary negligence of Dancing Stars Dance Studio or any person listed above.
To participate in this activities and to use the equipment and facilities, I further agree to indemnify and hold harmless Dancing Stars Dance Studio from any and all claims which are brought by or on behalf of the minor and/or responsible parent/guardian and which are in any way connected with such use of participation of the minor and/or responsible parent/guardian/party.
Permission is given Dancing Stars Dance Studio permission to use mine and my child’s image on print, online, digitally, on any and all social medias for marketing and promotional purposes during and after enrollment at Dancing Stars Dance Studio.
