FIRST AID & MEDICAL AUTHORIZATION
Medical Authorization & First Aid Consent
I understand that participation in dance, acrobatics, tumbling, and related activities involves physical activity and the possibility of injury. I certify that my child is physically able to participate in these activities.
In the event of an illness or injury, I authorize Next Level Dance Company staff to administer basic first aid, including but not limited to cleaning wounds, applying bandages, ice packs, and other routine first aid measures deemed appropriate until I can be contacted.
If emergency medical treatment is necessary and I cannot be reached, I authorize Next Level Dance Company to seek emergency medical care for my child, including transportation by emergency medical personnel if deemed necessary.
I understand that I am financially responsible for any medical expenses incurred.
I agree to notify the studio of any medical conditions, allergies, medications, or physical limitations that may affect my child's participation.

Child Safety & Protection Policy
The safety and well-being of every student is our highest priority.
Next Level Dance Company is committed to providing a safe, respectful, and positive environment for every child To help ensure student safety:
• Staff members will treat every student with dignity, respect, and professionalism.
• Appropriate physical corrections will only be used when necessary for dance instruction and injury prevention, and always in a professional manner.
• Private one-on-one meetings between staff and students will be avoided whenever possible.
• Students should never leave the studio with anyone other than an approved parent, guardian, or emergency contact.
• Bullying, harassment, discrimination, intimidation, or inappropriate behavior by students, parents, or staff will not be tolerated.
• Any concerns regarding student safety should be reported immediately to studio management.
• Next Level Dance Company reserves the right to dismiss any student or family whose behavior threatens the safety or well being of others.
