2026-2027 Waiver and Release of Liability

WAIVER AND RELEASE OF LIABILITY
I understand that my enrollment and attendance and/or my child’s enrollment and attendance at events, classes, competitions and sessions related to or organized by Center Stage Dance Academy, are strictly voluntary. I also understand that the novel coronavirus (COVID-19) has been labeled by the highly contagious and is subject to ongoing evaluation and research. Therefore, based on other representations and studies that have been given or in consideration for the Studio permitting me and/or my child or ward to be enrolled in and attend events, classes, competitions and sessions related or organized by the Studio.
Acceptance of Risk; Release/Indemnification. I am fully aware that there are a number of risks associated with me and/or my child or ward entering into Studio property or participating in Studio events (in person or online) and/or sessions and/or competitions or other Studio business which could result in physical or emotional injury, paralysis, death, or damage to me, to property, or to third parties. I understand that such injuries or outcomes may arise from my own or others’ negligence, conditions related to travel, or the condition of the Studio premises. Nonetheless, I assume all related risks, both known and unknown to me, of my participation in this activity. I hereby release, indemnify, and hold harmless the Studio, its owners, officers, directors, employees, agents, representatives, successors and assigns, I knowingly and freely assume all such risks, both known and unknown, relating to me, my child and/or ward’s participation in or attendance at Studio events, classes or sessions, and I hereby forever release, waive, relinquish, discharge, and covenant not to sue the Studio, its owners, officers, directors, employees, agents, representatives, and their successors and assigns (collectively, “Studio Representatives”), from any and all claims, actions, liabilities, rights, damages, costs, or expenses of any kind whatsoever (“Damages”) arising out of or relating to my and/or my child or ward’s enrollment, attendance, or participation at Studio events, classes, competitions or sessions, whether arising from the negligence of the Studio or Studio Representatives or otherwise.
Covenant Not to Sue. I further promise not to sue the Studio or Studio Representatives for Damages described above.
I hereby authorize Center Stage Dance Academy to publish photographs or videos taken of my student or myself for use in marketing materials.
