Participant Waiver, Release & Parent/Guardian Agreement
By registering myself and/or my child with DeStella Dance Centre (DDC), I acknowledge and agree to the following policies, terms, and conditions for participation in classes, rehearsals, performances, competitions, conventions, workshops, camps, and other studio-related activities.
ASSUMPTION OF RISK & ACCIDENT/INJURY WAIVER
I understand that participation in dance, tumbling, acrobatics, baton, conditioning, stretching, rehearsals, performances, and other physical activities involves inherent risks.
These risks may include, but are not limited to, slips, falls, collisions, sprains, strains, fractures, muscle injuries, head injuries, or other accidental injuries.
I understand that DeStella Dance Centre and its instructors will take reasonable precautions to provide a safe environment; however, accidents and injuries may still occur.
I voluntarily assume the risks associated with participation and agree that DeStella Dance Centre, its owner, instructors, employees, assistants, volunteers, and representatives will not be held responsible for injuries or accidents arising from the ordinary risks of participation, to the fullest extent permitted by law.
MEDICAL EMERGENCIES
In the event of an injury or medical emergency, I authorize DeStella Dance Centre staff to provide basic first aid and to contact emergency medical services when reasonably necessary.
If I cannot be reached, I authorize appropriate emergency medical treatment for my dancer.
I understand that I am financially responsible for any medical treatment, ambulance transportation, hospital care, or other medical expenses incurred on behalf of myself or my child.
Parents/guardians are responsible for keeping all emergency contact and relevant medical information in the Parent Portal current.
PHOTO, VIDEO & SOCIAL MEDIA RELEASE
I understand that photographs and videos may be taken during classes, rehearsals, studio events, competitions, performances, conventions, fundraisers, community events, and other DDC activities.
I give DeStella Dance Centre permission to photograph and/or record my dancer and to use appropriate images or videos for studio-related purposes, including:
DeStella Dance Centre social media pages
The DDC website and Parent Portal
Promotional materials and advertisements
Flyers, posters, programs, and studio displays
Recital, competition, and event-related content
Other appropriate studio marketing and promotional purposes
DDC will not intentionally publish a dancer's private contact information or other sensitive personal information.
Parents/guardians who do not want their dancer photographed or included in DDC social media or promotional materials must notify the studio in writing.
SOCIAL MEDIA CONDUCT
Dancers and families are expected to use good judgment when posting content involving DeStella Dance Centre, its dancers, teachers, staff, and families.
Bullying, harassment, threatening language, inappropriate comments, or intentionally harmful or disrespectful posts directed toward DDC dancers, families, faculty, or the studio will not be tolerated.
Private studio information, choreography, team information, or other material specifically identified by DDC as confidential should not be posted publicly without permission.
PERSONAL PROPERTY
DeStella Dance Centre is not responsible for lost, stolen, misplaced, or damaged personal belongings.
Dancers should avoid bringing unnecessary valuables to the studio.
Cell phones must remain on silent during class and should not interfere with instruction. Jewelry should not be worn during class unless specifically permitted by an instructor. DDC is not responsible for phones, jewelry, glasses, dancewear, shoes, costumes, toys, or other personal possessions.
FACILITY & EQUIPMENT
Dancers may only use tumbling equipment, props, barres, studio equipment, or other specialized equipment when instructed or given permission by a DDC faculty member.
Running, horseplay, unsafe use of equipment, or behavior that creates an unnecessary risk of injury is prohibited.
PARENT/GUARDIAN RESPONSIBILITY
Parents/guardians are responsible for ensuring that dancers are appropriately dressed and prepared for class and are dropped off and picked up in a timely manner.
Parents/guardians are also responsible for reviewing studio communications, Parent Portal announcements, schedules, policies, and the Season 14 Handbook.
CODE OF CONDUCT
DDC is committed to providing a positive, respectful, and safe environment.
Dancers and parents/guardians are expected to treat teachers, staff, dancers, other families, facilities, and property with respect.
Disrespectful, disruptive, threatening, bullying, or inappropriate behavior may result in disciplinary action and, when necessary, removal from a class, activity, team, or the studio.
ILLNESS & HEALTH
Dancers should not attend class when they are experiencing symptoms of a contagious illness or when participation would be unsafe for themselves or others.
Parents/guardians are responsible for determining whether their dancer is healthy enough to participate and for notifying DDC of relevant restrictions or accommodations that the studio needs to know in order to safely conduct class.
STUDIO POLICIES
By registering with DeStella Dance Centre, I acknowledge that I am responsible for reviewing and following the policies contained in the Season 14 Handbook and Parent Portal, including applicable policies regarding attendance, tuition and fees, costumes, recital participation, studio conduct, class placement, competition participation, and other studio procedures.
ACKNOWLEDGMENT & ELECTRONIC AGREEMENT
By checking the acknowledgment box and/or electronically accepting this waiver during online registration, I confirm that:
I have read and understand this Participant Waiver, Release & Parent/Guardian Agreement.
I understand that participation in dance and related physical activities carries inherent risks of injury.
I voluntarily allow myself and/or my child to participate in DeStella Dance Centre activities.
I agree to follow DeStella Dance Centre policies and procedures.
I understand that my electronic acceptance of this agreement is intended to serve as my acknowledgment and agreement to these terms.
