Participation & Liability Waiver

1. Participation & Assumption of Risk
By enrolling in classes or activities with Madison Avenue Dance Studio, LLC and/or Shine Dance Competition (collectively referred to as “MADS”), I understand that dance and related activities involve physical movement and inherent risks.

These activities include, but are not limited to:

Recreational classes
Competitive teams
Camps and intensives
Rehearsals
Performances and competitions
Workshops and master classes
Studio rentals
Special events
Off-site events and travel related to participation

I understand that participation may involve risks such as falls, physical exertion, collisions, equipment use, transportation risks, or other known and unknown risks that could result in injury, illness, emotional distress, property damage, or in rare circumstances, serious injury.

I voluntarily assume all risks associated with participation, including those that may arise from the negligence of MADS.

2. Release of Liability
On behalf of myself and/or my minor child, I hereby release, waive, discharge, and agree not to bring legal action against Madison Avenue Dance Studio, LLC, Shine Dance Competition, and their owners, directors, employees, instructors, contractors, volunteers, and representatives for any claims arising out of participation in studio activities, including claims resulting from ordinary negligence.

3. Medical Authorization
In the event of an injury or medical emergency, I authorize MADS staff to seek emergency medical treatment for my child if I cannot be reached.

I understand that I am financially responsible for any medical care provided and that MADS does not provide medical insurance coverage for participants.

4. Insurance Responsibility
I acknowledge that it is my responsibility to maintain adequate health insurance coverage for myself and/or my child participating in studio activities.

5. Photo & Media Release
I grant permission to Madison Avenue Dance Studio and Shine Dance Competition to use photographs and/or video recordings of my child (or myself, if an adult participant) for promotional purposes including social media, website content, marketing materials, advertising, and studio publications.

No compensation will be provided for such use.

6. Governing Law
This agreement shall be governed by the laws of the State of South Dakota.

7. Severability
If any portion of this agreement is found to be unenforceable, the remaining sections shall remain in full effect.

Acknowledgment
By signing this form, I confirm that:

I have read and understand this agreement.
I understand that I am giving up certain legal rights.
I sign this agreement voluntarily and without pressure
