1. Liability & Release of Liability Waiver
Purpose: Protects the studio if a student is injured.
Liability Waiver and Release
I, the undersigned, acknowledge that participation in dance classes, rehearsals, and related activities involves risks, including physical injury.
I agree to assume all risks and release Modern Dance Collective and its instructors, employees, and volunteers from any liability for injury, illness, or property damage that may occur during participation.
I understand that participation is voluntary and that I am responsible for my own safety.
I also authorize the studio to provide or seek emergency medical care if necessary.

2. Photo & Video Release
Purpose: Grants permission to use images/videos for promotion.

I give permission for Modern Dance Collective to photograph or record my child/myself during classes, performances, or events.
I understand these images may be used for marketing, social media, and promotional materials.
I release the studio from any claims related to the use of these images.

4. Refund & Cancellation Policy Acknowledgment
Purpose: Ensures students/parents understand financial policies.

All classes have a 2-month minimum enrollment.
To withdraw a student, you must email the studio before the 1st of the month to avoid being charged for the following month.

5. Code of Conduct / Behavior Agreement
Purpose: Sets expectations for students and parents.
Code of Conduct
Students and parents will treat instructors and classmates with respect.
Students will follow studio rules, including dress code, punctuality, and safety guidelines.
Disruptive behavior may result in removal from class without refund.
