Clean Heart Creatives LLC – Carissa Brooke
Music, Dance, & Artist Development Agreement
This Agreement (“Agreement”) is entered into between Clean Heart Creatives LLC (“Instructor”) and the undersigned student, parent, or legal guardian (“Student/Guardian”). By signing this Agreement, Student/Guardian acknowledges and agrees to the following terms and policies.
SERVICES
Clean Heart Creatives LLC provides personalized music, dance, and artist development lessons and classes tailored to each student’s individual and/or group goals. Instruction is designed to encourage growth, creativity, confidence, and skill development in a positive and supportive environment.
Students may be offered opportunities to perform what they have been working on during winter and/or spring showcases.
Disclaimer: Instructor does not guarantee any specific performance, audition, academic, or career outcome. Student progress depends on attendance, participation, personal practice, consistency, and commitment.
The Winter Showcase will be held at the studio. The Spring Showcase may be held at a larger offsite venue, to be determined.
OPEN / SERIES CLASSES
Open/Series Classes are weekly music, dance, or artist development classes that occur on an ongoing basis throughout the season. These classes are planned and structured by the Instructor with the expectation of regular weekly attendance.
Students are encouraged to enroll at the beginning of the Winter or Spring session; however, enrollment may remain open depending on class capacity and instructor approval.
Open Class Scheduling, Attendance, & Payments
Tuition reserves the student’s place in class and covers instructor preparation, scheduling, and studio operations. Tuition is not based solely on attendance.
Monthly tuition is due by the first class of each month unless otherwise agreed upon in writing. Monthly tuition may include 3–5 weeks depending on the calendar month and studio schedule.
No refunds, credits, or prorated tuition will be provided for missed classes, personal scheduling conflicts, vacations, illness, travel, or other absences.
If a student misses class, they may request to attend another age/level appropriate open class as a makeup class with prior Instructor approval and only if space permits. Makeup classes are not guaranteed and remain at the sole discretion of the Instructor.
Studio closures due to holidays, scheduled breaks, or seasonal closures are not considered canceled classes and are accounted for within the overall tuition structure and yearly calendar.
If students choose to participate in the Spring Showcase, additional performance-related fees may apply, including costumes, attire, or venue-related expenses. Performance-related fees will not exceed $100 unless otherwise communicated in advance.
PRIVATE & GROUP LESSONS
Private and Group Lessons are scheduled in advance by mutual agreement between Instructor and Student/Guardian.
Private/Group Scheduling, Attendance, & Payments
Lessons are to be paid in advance on a weekly or monthly basis, as agreed upon between parties.
Payments are non-refundable except in the case of Instructor cancellation.
A minimum of 24 hours’ notice is required to cancel or reschedule a private or group lesson without penalty.
Late Cancellation or No-Show
Lessons canceled with less than 24 hours’ notice, or lessons missed without notice, will be charged in full and are not guaranteed to be rescheduled.
Instructor Cancellation
If the Instructor must cancel a lesson, the lesson may be rescheduled, credited, taught virtually, covered by a substitute instructor, or otherwise reasonably accommodated at the Instructor’s discretion.
LATE PAYMENTS & UNPAID BALANCES
A late fee of $15 may be applied to overdue balances not received within 7 days of the payment due date.
Clean Heart Creatives LLC reserves the right to pause or discontinue lessons/classes for accounts with unpaid balances.
Returned payments, failed automatic payments, declined cards, or chargebacks may incur additional processing fees.
WITHDRAWAL & DISCONTINUATION
For ongoing classes or recurring lessons, Student/Guardian agrees to provide at least 14 days written notice before withdrawing from services.
Tuition and payments already made are non-refundable and non-transferable.
Clean Heart Creatives LLC reserves the right to suspend or terminate services at any time due to non-payment, excessive absences, scheduling conflicts, disruptive behavior, inappropriate conduct, safety concerns, or if the Instructor determines the program is no longer an appropriate fit for the student.
BEHAVIOR & CONDUCT
Respectful, safe, and appropriate behavior is expected from all students, parents, and guardians.
Disruptive, disrespectful, unsafe, aggressive, inappropriate, or repeatedly distracting behavior from either a student or parent/guardian may result in suspension or termination of services at the Instructor’s discretion.
Clean Heart Creatives LLC reserves the right to deny or discontinue services at any time if the learning environment, student safety, or studio culture is compromised.
CHILD SAFETY & GUARDIANSHIP
For students under 16 years old, only pre-approved family members or approved individuals may pick up the student. Instructor must be notified in advance if someone other than the approved guardian will be transporting the child.
For students under 5 years old, a parent or legal guardian must remain on the premises for the duration of the lesson or class.
Parents/Guardians are responsible for ensuring timely drop-off and pickup of students.
HEALTH, INJURY, & LIABILITY
Participation in music, dance, movement, and artist development instruction involves physical activity and carries an inherent risk of injury.
By participating, Student/Guardian voluntarily assumes all risks associated with participation in lessons, classes, rehearsals, showcases, performances, or related activities.
Clean Heart Creatives LLC, its instructors, staff, contractors, and affiliates shall not be held liable for injuries, accidents, illnesses, lost or stolen items, property damage, or other incidents occurring during or related to participation in services.
Student/Guardian affirms that the student is physically able to participate and agrees to inform the Instructor of any injuries, medical conditions, allergies, behavioral concerns, or limitations that may affect participation or safety.
EMERGENCY MEDICAL AUTHORIZATION
In the event of a medical emergency, Instructor may seek reasonable emergency medical care for the student if a parent/guardian cannot be reached immediately.
Any medical expenses incurred remain the responsibility of the Student/Guardian.
WEATHER, EMERGENCIES, & FORCE MAJEURE
In the event of severe weather, natural disasters, illness outbreaks, power outages, government restrictions, emergencies, or circumstances beyond the Instructor’s control, lessons/classes may be rescheduled, moved online, credited, shortened, or otherwise reasonably adjusted at the Instructor’s discretion.
COMMUNICATION
Student/Guardian is responsible for reviewing studio emails, text messages, announcements, invoices, scheduling updates, and policy communications.
Important information regarding tuition, scheduling, showcases, studio closures, and policies will primarily be communicated electronically.
STUDENT/TEACHER FIT & REFERRALS
Clean Heart Creatives LLC specializes in personalized and creative instruction; however, the Instructor may not be the best fit for every student.
If at any time the Instructor determines that another teacher, program, or learning environment may better support the student’s needs, goals, personality, or skill level, the Instructor reserves the right to discontinue services and may provide referrals when appropriate.
MEDIA CONSENT (OPTIONAL)
