Liability Waiver and Photo/Video Release Form
I, [Parent/Guardian], the undersigned, hereby acknowledge and agree to the following terms and conditions regarding my child's participation in dance classes, activities, events, and performances at BiZi Dance Company.
Assumption of Risk: I understand that dance activities involve physical exertion and potential risks, including but not limited to, personal injury, illness, or damage to personal property. I accept all risks associated with my child's participation in dance classes and related activities.
Tuition and Payments: I understand that any registration fees are nonrefundable. I also understand that my tuition is due on the 1st of every month with a grace period through the 5th. I also understand that if my tuition is not paid by the 5th, I will have an automatic late fee of $25 charged to my account. BiZi Dance Company has the right to prevent dancers from participating in classes, rehearsals, or performances if their account balance is not current. If a balance remains unpaid as of the 15th of the month, dancers will be asked to sit out until the account is brought up to date.
Release of Liability: In consideration of my child being allowed to participate in dance classes and activities at BiZi Dance Company, I, on behalf of myself, my child, and our respective heirs, executors, administrators, and assigns, hereby waive, release, and forever discharge BiZi Dance Company, its owners, instructors, employees, and representatives from any and all liabilities, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness that may be sustained by my child or me while participating in dance activities.
Photo and Video Release: I grant BiZi Dance Company and its authorized representatives the irrevocable right to use photographs, videos, and audio recordings of my child taken during dance classes, activities, events, and performances for promotional, advertising, and educational purposes, including but not limited to the studio's website, social media, brochures, newsletters, and other marketing materials.
I acknowledge that I have read and understood this Liability Waiver and Photo/Video Release Form, and I voluntarily agree to its terms. I intend for this document to be legally binding and to apply to all present and future dance classes, activities, events, and performances at BiZi Dance Company
