DIGITAL CONTENT POLICY
House of Dance Twin Cities a full rights, title and interest to all images, photos, content, videos, logos/watermark, and any and all copies, derivations, edits or likenesses thereof, regardless as to whether copyrighted (collectively, the "Digital Content"). The redistribution, duplication, sampling, or sharing of any Digital Content is strictly prohibited, and nothing contained in this Agreement or otherwise shall be construed to grant any license, right, title or interest to use Digital Content without the express written consent of House of Dance Twin Cities, for and with due consideration.
PHOTO/VIDEO RELEASE
House of Dance Twin Cities may include Digital Content of students, instructors, and program activities on its website, social media sites, and any other promotional, sales, or marketing materials (collectively,
"HOD Channels"), and we/l therefore hereby give permission to House of Dance Twin Cities to include such Digital Content in HOD Channels and do not claim any right, title and interest thereto. I understand that this Digital Content will be accessible to anyone with internet access; however, no names of subjects will be published.
ATTENDANCE POLICY
I understand that per paid session, if I miss a class, 1 am voluntarily forfeiting said paid class. Any exceptions are at the sole discretion of Jake Riley and, if so granted, shall not affect future forfeiture of class fees. All aspects of the attendance policy in this paragraph apply equally to any live session, or any video otherwise created, provided by, or distributed by House of Dance Twin Cities.
RELEASE OF LIABILITY
I have read this registration completion form and acknowledge that by signing this form, I am subject to the terms and conditions of the Release of Liability (the "Release"), which is hereby incorporated by reference. This Release includes without limitation the use of or participation in any live session, or any video otherwise created, provided by, or distributed by House of Dance Twin Cities, regardless as to whether said use or participation occurs on the premises of House of Dance Twin Cities.
PARENT OR GUARDIAN FOR MINORS (UNDER 18 YEARS OF AGE)
The undersigned parent and/or natural guardian or legal guardian does hereby
represent that he/she is, in fact, acting in such capacity and agrees to this
Registration Form and the Release of Liability on behalf of the minor.
