I, the undersigned, acknowledge and understand that participation in any dance class, workshop, rehearsal, performance, pop-up class, or activity conducted by Shehnaaz Dance Academy LLC (SDA) may involve physical exertion and carry inherent risks of injury. I voluntarily assume full responsibility for any injury, accident, or loss that may occur to me or my child/ward while participating in such activities.
In the case of pre-existing physical, neurological, or mental health conditions, I understand it is my sole responsibility to consult with a physician prior to participation. I further agree that it is my responsibility to modify or refrain from any movements or exercises during class as needed.
This release of liability also extends to third-party venues where SDA activities, classes and events may occur. This agreement is entered into voluntarily and without duress and is intended to be legally binding. It shall be interpreted in accordance with its plain meaning and the purpose for which it was entered.
I hereby release, waive, and discharge Shehnaaz Dance Academy LLC and its owners, heirs, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability, claims, demands, or causes of action arising from or related to injury, illness, loss, or death incurred during or resulting from participation in any SDA programs or events.
By signing this waiver, I release Shehnaaz Dance Academy LLC. and its employees, affiliates and contractors from all claims derived from any injury/sickness that may be sustained by me or my party while attending any rehearsal, event, or outside performance. This Waiver is intended to be as broad and inclusive as is permitted by the laws of the State of New Jersey.
Make-Up Policy
SDA allows up to 2 make-up classes per student per semester, which must be completed before Spring Break/Thanksgiving Break. No make-ups will be granted after these breaks. For planned travel, please email SDA at least 48 hours in advance with details of the classes you will miss and your requested make-up date.
Photo & Video Release
I grant Shehnaaz Dance Academy LLC and its authorized representatives the irrevocable right to use photographs, videos, and/or audio recordings of me or my child/ward taken during classes, rehearsals, performances, or events for promotional, educational, or marketing purposes. These may include but are not limited to use on the SDA website, social media, flyers, and newsletters.
