Students of Devine Performing Arts LLC ("DPA") may have the opportunity to be photographed or videotaped at various dance events and activities (including, but not limited to, classes, competitions, rehearsals, performances, workshops, lessons, online/virtual classes, and/or other special programs).
With permission from a parent/legal guardian, these photos or videos may be included for out DPA publications or advertisements, including our website and various social media sites.
-To ensure privacy, specific names of children will not be included.
-All live streaming via Zoom will be password protected.
-No financial compensation shall be given for use of any photographs or video.
I acknowledge that DPA may choose not to use my photo or video at this time, buy may do so at its own discretion at a later date. DPA reserves the right to discontinue use of photos or video without notice.
I have read this release and I understand its contents. I consent to and authorize the use by DPA, or anyone authorized by DPA, of any and all photographs or video which have been taken of my child/children.
I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT.
