Participant Waiver & Release of Liability

By signing this form, I acknowledge and agree to the following on behalf of myself or my child:
Assumption of Risk:
I understand that participation in dance classes (including but not limited to ballet, modern, contemporary, and jazz) involves inherent physical risks, including but not limited to strains, sprains, falls, collisions, and other potential injuries. I voluntarily assume all such risks associated with participation.
Physical Fitness and Medical Conditions:
I certify that I (or my child) am physically fit to participate in dance classes and have not been advised otherwise by a medical professional. I agree to notify Corvallis Ballet Collective of any medical or physical condition that may affect safe participation.
Waiver and Release of Liability:
In consideration of participation in programs offered by Corvallis Ballet Collective, I hereby release, waive, and hold harmless the organization, its instructors, staff, directors, volunteers, and property owners from any and all liability, claims, or causes of action arising out of or related to any injury, illness, or loss that may occur while participating in classes, rehearsals, performances, or related events.
Emergency Medical Treatment:
In the event of an emergency, I authorize Corvallis Ballet Collective staff to obtain medical treatment for myself (or my child) and agree to be financially responsible for any costs incurred as a result of such treatment.
Photo and Video Release:
I grant permission to Corvallis Ballet Collective to photograph or video myself (or my child) during classes, rehearsals, and events for promotional, educational, or archival purposes. I understand that images may be used on the organization’s website, social media, brochures, or other materials.
*If I do not wish for images or videos of my child to be used, I understand that I must notify Corvallis Ballet Collective staff in writing. Please email us!
Code of Conduct and Policies:
I agree to follow all rules, safety protocols, and policies set forth by Corvallis Ballet Collective. I understand that failure to do so may result in removal from classes or programs.
ACKNOWLEDGMENT & SIGNATURE
I have read this waiver and release of liability in full and understand its terms. I understand that by AGREEING to this document, I am giving up legal rights, including the right to sue. I sign it freely and voluntarily.
