1. Acknowledgment of Risk
I understand that participation in dance classes including, but not limited to, ballet, tap, jazz, cheer, lyrical, and related dance or fitness activities involves physical movement and exercise that may carry risk of injury. These risks may include, but are not limited to, muscle strains, sprains, falls, collisions, or other physical injuries.
2. Assumption of Risk
I voluntarily allow my child to participate in dance classes, rehearsals, performances, and related activities offered by Leaping For Joy Dance Studio and acknowledge that participation is done at our own risk.
3. Release of Liability
I, on behalf of myself and my child, hereby release, waive, and discharge Leaping For Joy Dance Studio, its owners, instructors, staff, volunteers, and affiliates from any and all liability, claims, demands, damages, or causes of action arising from injury, illness, accident, or property loss that may occur while my child is participating in studio classes, rehearsals, performances, events, or while present on studio premises.
4. Medical Authorization
In the event of an emergency, I authorize staff of Leaping For Joy Dance Studio to seek emergency medical care for my child if I cannot be reached immediately.
I understand that I am responsible for any medical expenses incurred.
5. Behavior & Safety Policy
Students are expected to follow all studio rules and demonstrate respectful and safe behavior toward instructors and other students. Leaping For Joy Dance Studio reserves the right to dismiss a student from class or the program if behavior is unsafe, disruptive, or inappropriate.
6. Privacy Policy
Leaping For Joy Dance Studio respects the privacy of our students and families. Personal information collected on this form (such as names, addresses, phone numbers, and medical information) will be used solely for studio communication, registration, emergency contact, and administrative purposes.
We do not sell, rent, or share personal information with third parties except when necessary for emergency medical care or when required by law. Information is stored securely and only accessible to authorized studio staff.
