LIABILITY DISCLAIMER CORE DANCE CENTER AND THE INSTRUCTORS ARE NOT LIABLE FOR PERSONAL INJURIES, LOSS OF AND/OR DAMAGES TO PERSONAL PROPERTY. EACH STUDENT MAY DECLINE TO PARTICIPATE IN ANY ACTIVITY. PLEASE INFORM THE INSTRUCTOR OF ANY PHYSICAL LIMITATIONS YOUR CHILD MAY HAVE. IF YOU HAVE ANY DOUBT IN YOUR CHILD’S ABILITIES, PLEASE CONSULT WITH HIS/HER PHYSICIAN BEFORE PARTICIPATING. CORE DANCE CENTER CANNOT DISPENSE ASPIRIN OR ANY OTHER MEDICATIONS WITHOUT THE PARENTS CONSENT.
PHOTOGRAPHIC CONTRACT I HEREBY CONSENT TO AND AUTHORIZE THE USE AND REPRODUCTION BY CORE DANCE CENTER, OR ANYONE AUTHORIZED BY CORE DANCE CENTER, OF ANY AND ALL PHOTOGRAPHS THAT HAVE BEEN TAKEN OF MY CHILD(REN) FOR DANCE PURPOSES. CORE DANCE CENTER RESERVES THE RIGHT TO USE THESE PHOTOGRAPHS IN ANY OF ITS PRINT OR ELECTRONIC PUBLICATIONS.
I hereby acknowledge that I have read and understand the terms of the Liability Disclaimer, the Photographic Contract, the Studio Policies and Billing Procedures. I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms. I understand that Core Dance Center does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Core Dance Center, and I voluntarily assume the risks associated with such participation.
