Liability Waiver

I hereby release Storm Studio, employees/independent contractors, from all liability for personal injury, illness, or property damage occurring on or off the studios premisses. I understand that a physician should approve any form of exercise and certify that my dancer is in good health and capable of participating in physical activities. The risk of the dance classes is fully understood. I understand I am to maintain insurance or pay for medical expenses incurred. In case of emergency I understand that every effort will be made to contact me and/or the emergency contact listed. In the event that I or the emergency contact listed cannot be reached, I authorize Storm Studios to seek medical treatment at the nearest licensed health care facility to secure proper treatment and/or they may call paramedics and discharge my dancer to an ambulance if I am not able to authorize it.
I hereby give permission to Storm Studios to take and use photographs and videos of me/my dancer/my property in connection to Storm Studios and use them for promotional uses for the studio.
I understand that all payment is non-refundable. I understand my dancers spot in class is not reserved until registration payment is received by Storm Studios. I have read and agree to Storm Studios 25/26 Studio Handbook.
