Dance Envy Inc. Consent and Liability Waiver

I understand and agree that in participating in any dance class, workshop, rehearsal, or performance at Dance Envy Inc., there is a possibility of physical injury. I voluntarily assume all risks and responsibility for any such injury or accident which might occur to me or my child during any classes, rehearsals, performances, or activities conducted by Dance Envy Inc.

I hereby release, waive, discharge, and hold harmless Dance Envy Inc., its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability, claims, demands, or causes of action whatsoever for any damage, loss, or injury, including but not limited to bodily injury, medical expenses, or property damage, that may arise out of or in connection with participation in any classes or activities conducted by Dance Envy Inc., whether such injury occurs on or off the premises.

I further agree to indemnify and hold harmless Dance Envy Inc. from any claims, damages, costs, or expenses arising from my own or my child’s participation. I acknowledge that I, or my child, should be aware of our own physical limitations and agree not to exceed them.

If I am signing this waiver for my child, I certify that I am the parent or legal guardian and have the authority to waive these rights on behalf of my child.

Additionally, I grant permission for Dance Envy Inc. to use photographs or videos of students for promotional and publicity purposes, including but not limited to social media, website, and marketing materials.

I have read, understand, and voluntarily agree to the terms and conditions of this waiver.
