Participation Waiver

Participation Waiver: Please read this form carefully and be aware that in signing up and participating in class(es), you will be waiving and releasing all claims for injuries/illness you might sustain arising out of the class(es). As a participant in the class(es), I recognize and acknowledge that there are certain risks of physical injury as well as illness such as SARS-COV-2 and I agree to assume the full risk of any injuries, illness, damages, or loss which I may sustain as a result of participating in any and all activities with or associated with such class(es). I agree to waive and relinquish all claims I may have as a result of participating in the class(es) against Quality Music Center Ltd. and its officers, agent, servants and employees. I do hereby release and discharge Quality Music Center Ltd. and its officers, agents, servants and employees from any and all claims from injuries, illness, damage or loss which I may have or which may accrue to me on account of my participation in the class(es). I further agree to indemnify and hold harmless and defend Quality Music Center Ltd. and its officers, agents, servants and employees from any and all claims resulting from injuries, illness, damages and losses sustained by me and arising out of, connected with, or in any way associated with the activities of the class(es).
While our studio complies with the State Health Department and the Centers for Disease Control and Prevention Infection Control guidelines to prevent the spread of the SARS-COV-2 Virus, we cannot make any guarantees. Our staff are symptom-free and, to the best of their knowledge, have not been exposed to the virus. However, since we are a place of public accommodation, other persons (including other patrons) could be infected, with or without their knowledge. In order to reduce the risk of spreading SARS-COV-2, we have asked you several “screening” questions below. For the safety of our staff, other patrons, and yourself, please be truthful and candid in your answers.
I authorize Quality Music Center Ltd. and The Children's Theatre of Chesapeake to use any video, photo or likeness of my child (me) for advertisement purposes such as brochures, the Quality Music, & Dance websites, Quality Music & Dance Facebook Page or mailings. The student’s name will NEVER be used in conjunction with the video/photo.
IN SIGNING THIS AGREEMENT, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing Waiver of Liability, Hold Harmless Agreement, Photo-Video Release as well as The Policies and Procedures. I understand it and sign voluntarily as my own free act and deed; no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made; I am at least eighteen(18) years of age and fully competent; and I execute this Agreement for full, adequate and complete consideration fully intending to be bound by same.
