RELEASE OF LIABILITY
I recognize and understand the risks of physical injury inherent to dance and dance training and I fully assume those risks. As the legal parent or guardian, I voluntarily release and hold harmless Pasitos Dance and Music Academy, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Pasitos Dance and Music Academy, its owners and operators or in route to or from any of said premises. The undersigned gives permission to Pasitos Dance and Music Academy, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.
MEDICAL ATTENTION
In case of physical injury or medical emergency, I hereby authorize Pasitos Dance and Music Academy to make necessary arrangements to transport myself or my child [if no parent or guardian present] to a medical treatment facility as necessary. All such transportation and medical treatment will be at my sole cost and expense. In extreme emergency, or if my child is under 18 years of age, I understand that Pasitos Dance and Music Academy will attempt to notify the person(s) I have named as my emergency contact(s) of my condition and how to reach me.
VISITORS AND GUESTS
Visitors and guests are not permitted. The only adults permitted in the studio are those that are listed in each family’s participation account. Each adult who plans to participate or be present must be added as a “student” in the online portal. Go to the online portal → under Account click on Students → Under “Add Students to your Family” add each adult that may participate or be present. This is to maintain a record of who is in proximity of the children participating. Guests that are not documented in our academy enrollment forms are prohibited from loitering in the lobby or outside of class. There are TV monitors located in the lobby of the studio for parents/guardians to observe their children.
PROTECTION OF PROPERTY
I understand and agree that it is my sole responsibility to safeguard my personal property while attending or participating in any classes, rehearsals, workshops, or performances. I hereby release Fancy Feet Dance Academy, its employees and dance teachers from all liability for loss or damage to my personal property while attending or participating in classes, rehearsals, workshops, or performances. I also agree to abide by any rules, regulations and policies set forth by Fancy Feet Dance Academy.
PHOTO RELEASE
I authorize the Pasitos Dance and Music Academy to record and edit into Video/Photograph and related materials my name, likeness, image, voice, interview, and performance on our website. The Pasitos Dance and Music Academy Creative may use and authorize to use all or parts of the Video or Photograph. Pasitos Dance and Music Academy shall own all right, title, and interest in and to the Video or Photograph, including the recordings, to be used and disposed of without limitation as Pasitos Dance and Music Academy shall in sole discretion determine.
Pasitos Dance and Music Academy occasionally photographs classes and performances. These pictures may be shared at the academy, on our website and in other media including press releases, academy materials, and social media pages. To ensure privacy, the names of children will not be included. No financial compensation shall be given for use of any photographs. You acknowledge that Pasitos Dance and Music Academy may choose not to use photos at the time they are taken, but that they reserve the right to use photos in the future or to discontinue photos without notice, at their discretion. However, we respect the rights of any family or student who does not wish to have their images made public.
If you do NOT want photos of yourself or your child shared, please send an email within 24 hours of registration to pasitosacademy916@gmail.com with the subject line "No photo release" and we will not include your child in any photography that is made public.
INFORMATION PRIVACY
The privacy of your personal information is very important to Pasitos Dance and Music Academy. We will not sell, rent or trade your personal information to any third party. We will take special care to protect the privacy and security of your information. This policy explains our information practices and the way that Pasitos Dance and Music Academy collects and uses your information. This Privacy Policy covers the information you provide to us offline and at Pasitos Dance and Music Academy website.
INFORMATION COLLECTION AND UTILIZATION
When you register for classes at Pasitos Dance and Music Academy, we collect contact information such as guardian names, addresses, phone numbers, emergency contacts, email addresses and student information such as student names, birthdays, medical conditions, and your comments. Some of the information is required and is noted with a red asterisk on the forms. This information is needed to manage academy business and will not be used or sold for any other purpose, except as described below. Some of the information is optional (NOT marked with a red asterisk). Optional information is requested to help us better understand our customers and is used only as needed for academy business. This information will not be used or sold for any other purposes, except as described below.
INFORMATION ACCESS
When you register, you are required to enter an email address and a password. The combination of that e-mail account and password is used to ensure that only you have access to, and only you have the ability to modify, your personal information through Pasitos Dance and Music Academy's website. Pasitos Dance and Music Academy office personnel have access to your personal information only to manage the academy business. Pasitos Dance and Music Academy will access your personal information and/or share it with third parties only for the following purposes: (i) as needed for academy business; (ii) if required to do so by law or to comply with legal process; or (iii) to protect the rights, property, or personal safety of the public. If you have any questions about our Privacy Policy or if you wish to update or remove any of your personal information, please contact the office.
TUITION AND PAYMENTS
There is an annual Registration Fee of $35 that will be charged automatically as you enter each year of your membership. For example, if you register to begin classes on June 1, 2025, you will be charged for your annual registration fee on June 1 of each year thereafter. Tuition is automatically taken out of your account on the 1st of every month. Auto pay is the only tuition option. We do not prorate months for missed days or holidays. If your card is declined and payment is not received by the 5th of the month, you will receive a $10 late fee. Membership is month to month until cancellation with a 15-day written advanced notice. I understand that these fees are non-refundable and non-transferable. Monthly fees are processed through a secure program and parent portal called Studio Pro. All clients are to have one card on file or bank account connected for auto pay. Clients may set up autopay using a checking account with no additional monthly fee. Clients may instead set up a credit card payment for an additional monthly fee.
MEMBERSHIP
Membership is month to month until cancellation with a 15-day drop request. If you are dropping, you will need to send an email to pasitosacademy916@gmail.com 15 days prior to the 1st of the next month to stop the automatic payment for that following month. The only prorated days are for enrolling after the start of the month. We do not prorate months for missed days, holidays, academy closure, or school vacations.
MAKE-UP CLASSES
There are no make-up classes available at this time. This policy may change as the academy grows and offers more classes.
REFUNDS
We do not offer refunds. There are no refunds on the annual registration fees. There are no refunds on the monthly tuition payments once they have been processed. There is no credit given and/or refunds for class(es) missed due to dropped classes, holiday, vacation, illness, weather, etc.
AGREEMENT
I have carefully read and agree to adhere to the Pasitos Dance and Music Academy waivers and policies and will take the responsibility to follow them. I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms. Parent(s) or Court-Appointed Legal Guardian(s) must sign for any participating minor (those under 18 years of age) and agree that they and the minor are subject to all the terms of this document, as set forth above. I certify that I am the Parent or Legal Guardian of the above minor.
