The purpose of this consent form is to grant Jayme's Fitness & Dance permission to use student's personal information for the purpose of carrying out administrative and operational duties. Jayme's Fitness & Dance agrees to respect and protect any information that is given.
This includes Students'/Parents' names, health information, personal information, email and phone numbers.
Names, photos, and videos may be used and published in local newspapers and/or on Jayme's Fitness and Dance Website and/or Facebook page, on Radio or Television, or on other media, for the purposes of promoting Jayme's Fitness and Dance and its Students. Discretion will be used at all times when choosing any photos.
I hereby give my consent to all of the above.
