Cottage Dance Academy Studio Policies

• All dancers must sign an agreement by the beginning of their first class. Class placements are based on skill level, attitude, motivation, dedication, respect toward teachers and fellow dancers, and class/performance/attendance of each dancer based on the previous year. We do not place students on teams with their friends in mind and will not follow this request by dancers or parents. Each dancer’s abilities are different and are recognized individually.

• I understand that alternates may be appointed as seen fit on any performance/competition Teams. I understand that if I am chosen as an alternate, I can earn my spot in any routine by successfully learning all the choreography and performing them well and being present in class. ALL alternate dancers are required to attend class regularly, pay all tuition and fees, learn all material and be available for all performances and/or competitions. Dances performed and competed by alternates will be determined by that team's instructor.

• Competition team dancers must attend their required weekly classes, all performances and all competitions. Dates are posted in advance so that all dancers and parents have plenty of time to plan accordingly. If dancers don’t attend shows and competitions despite being informed of dates well in advance, their spot on the team can be revoked or changed to an alternate- this includes future spots on competition teams at auditions for the following year.

• Upon signing the TEAM CONTRACT, you must finish the entire dance year September-May.

• All dancers must learn and know their required choreography. Not doing so may result in that dancer’s removal from dances.

• Dancers and/or parents must responsibly give plenty of notice (1 + months) to the directors of any graded school activities that will require that dancer to miss practices. It is unacceptable to miss performances or competitions unless it involves a last-minute emergency, serious illness or injury. Missing practices or performances at the last minute because of sports or negligence is unacceptable and unfair to other members of your team and classes- this could lead to that dancer being replaced by an alternate. We are flexible, but if your sports/extra-curricular schedule interferes with classes too much you will be pulled from your team.

• All students must pay their tuition, costume fees and competition fees on time. If a dancer has issues paying any of these on time their spot on the team will be filled by a team alternate and they will no longer be able to compete, perform or practice on their existing team.



• All team dancers are required to purchase a studio jacket to be worn to dance events unless they have a jacket from a previous year that fits well. Warm-ups and accessories are optional to all dancers at Cottage Dance Academy for now. Dancers are required to purchase assigned leotards, tights, shoes, and other classroom uniforms at the beginning of the season.

• All team dancers and/or parents will make sure they are receiving and reading through all emailed newsletters. Not doing so can result in dancers missing payments, missing rehearsals or accidentally scheduling over major dance events which can also result in that dancer being pulled from dances and/or their team.


• All students are required to have a great attitude and be courteous towards their fellow dancers, instructor(s) and directors, respectful of the studio guidelines and be a positive representative of Cottage Dance Academy at performances and competitions. Not doing so will lead to not being invited back onto the team the following year.

• All parents/guardians must have a great attitude and be supportive of the dancers and the studio and stay up to date with all events going on throughout the year with Cottage Dance Academy. Parents/Guardians are expected to make sure their dancers attend all rehearsals, classes, performances and competitions required of them. Everything we do at Cottage Dance Academy is in the best interest of the dancers and although we push the dancers and each other to be better dancers, we need positive encouragement and consistency from all involved. Parents must also respect instructors, staff, director’s and other students. Not doing so will jeopardize your child’s position in the studio.

• We are a zero-tolerance studio when it comes to unnecessary drama. If you or your parent/family member cause a scene in class or at dance events or if we find out you or parent/family member has been bullying or harassing another student or staff member, your involvement with your team(s) and our studio will be immediately terminated. Above all else, a good attitude is what we need to make our teams thrive. It does not matter how long you have danced with us or what your skill level is, if your attitude is toxic, you can no longer be a part of Cottage Dance Academy.


• Extra rehearsals may be needed for any team leading up to any competition or performance. These will be held on Saturdays and all dancers must be in attendance.


• I agree to wear my costumes properly that have been fitted to me, to properly care for my team attire by keeping all costumes on hangers or in a garment bag at performances and competitions, and to follow my team(s) hair/makeup style and to arrive at all performances and competitions hair/makeup ready.

• Costumes (including head pieces and accessories) must be kept clean and tidy for all performances and competitions. Dancers are responsible for their own costumes. Please do not leave costumes in a heap or leave costumes behind at performances or competition venues. All costumes must be either hand washed or dry-cleaned but please ask first what is best for your particular costume. DO NOT wash costumes in the washer. Some costumes can be ironed or steamed but some cannot. If damage is done to your costumes, you are required to pay to replace it.


•. Hip-Hop shoes are part of a costume and class attire. These are not for outside use. Shoes should be cleaned up before entering the studio dance floor. If Hip-Hop shoes are damaged before competition season the dancer is responsible for buying new shoes or will not be able to compete in our team Hip-Hop dance.

• All dancers are required to pay their costume fees on time in order to have a costume for performances and competitions. You will not receive your costume until it is fully paid for. There may be a possible costume buy back at the end of the year. Unless specified costume fees are non-refundable.

• All dancers are required to attend all performances throughout the year. You will be notified which ones to perform at throughout the year as performances are scheduled. All teams are required to perform at our two mandatory studio performances each year; Ballet Nutcracker in December and our end of the year showcase in May. Performance Teams are required to perform at the annual Freeze Frame Dance Showcase at CSI in January unless otherwise announced and our year end showcase in May.

• All team parents are required to purchase proper personal dance attire for all dancers such as tights, leotards, socks, wig caps (If required with costume) shoes, earrings, dance tanks for boys and proper supportive dance undergarments for girls and boys.

• NON-COMPETE CLAUSE: I agree not to share any Cottage Dance Academy choreography with any other studio, director, or dancer, and will not recreate any Cottage Dance Academy choreography with anyone outside of Cottage Dance Academy during or after my time with CDA out of respect for the studio, directors, choreographers and instructors of Cottage Dance Academy without permission.



Commitment Form
We understand that we are registering for a Cottage Dance Academy. We understand that once we are selected, we are obligated to fulfill our membership on a team or in a class through the recital in May. Membership includes but is not limited to: attending required weekly classes, attending extra rehearsals, attending required workshops/conventions/choreography sessions, maintaining and building dance skills through the course of the summer, traveling out of state, making monthly installments for fees as it pertains to you, participating in some required fundraisers, have good attendance, good grades, a good work ethic, good sportsmanship and a positive attitude. If chosen for a team, Cottage Dance Academy team must be Prioritized, which means members are not allowed to participate with other private dance studios that conflict overly with practice, performances, Conventions, and competitions while a member of a Cottage Dance Academy’s Competition team. Public school dance teams are not considered a private studio dance team. However, we expect you to make the Cottage Dance Academy Dance Company your dance priority. We also understand that all decisions made by the Cottage Dance Academy faculty are final and are not to be disputed.

my signature below, I hereby grant Cottage Dance Academy LLC, its successors, licensees, and assigns, the non-exclusive right and license to use, re-use, publish, and republish my physical likeness in all photographs and videos described below that were taken of me by Cottage Dance Academy LLC, in whole or in part, severally or in conjunction with other photographs and videos, in any medium or form, now known or that may be hereafter known or devised, for any purpose and in connection with, without limitation, Cottage Dance Academy LLC portfolio, internal and external advertising, publicity, illustration, trade, sale, promotion, and exploitation thereof throughout the world. This license shall be without charge or royalty and for perpetuity. Cottage Dance Academy LLC shall have the right to use my name in connection with this license. I warrant and represent that I have the right to grant this license and that I am not a party to any contract or agreement that restricts the grant of this license. I hereby indemnify Cottage Dance Academy LLC and hold Cottage Dance Academy LLC harmless against all losses resulting from the breach of such warranty. I agree that I will not assert or maintain against Cottage Dance Academy LLC, its successors, assigns, and licensees a claim, action, suit, or demand of any kind or nature whatsoever, including without limitation those grounded upon copyright, trademark, invasion of privacy, rights of publicity or other civil rights, or for any other reason in connection with your use of the photographs. I hereby release Cottage Dance Academy LLC, its successors, assigns, and licensees from any and all such claims, actions, causes of action, suits, and demands whatsoever that I may now or hereafter have against Cottage Dance Academy LLC or them. Wherever the word “I” is used herein it shall include my successors and assigns.

HOLD HARMLESS AGREEMENT AND ASSUMPTION OF ALL RISKS
I, by acknowledgement and complete agreement below, hereby declare both personally and/or as parent and/or legal guardian of the child that is or maybe less than 18 years of age (either party hereafter referred to as I and/or We) the following:
So that I and/or We can enjoy activities for which I may not be skilled I and/or We are informed that there is some inherent unavoidable risks, known and unknown, that may cause harm, loss, personal injury and in the most extreme cases even potentially death, in participating in any course of physical activity. Some risks cannot be avoided without destroying the unique character of these activities and can also be the cause of loss or damage to others, equipment and/or others personal property. Certain risks are indigenous to physical contact sports that involve any form of the arts, dance and any other activity that student, client, or spectator willfully engages in relation to or in affiliation with Cottage Dance Academy LLC.
I and /or we expressly agree that I and/or we fully assume 100% complete and full responsibility for all risks described above and/or any of others known or unknown. And hereby hold harmless and release 100% of all and any liability and discharge Cottage Dance Academy LLC and/or each and every student, client, Owner, Operator, Officer, Partner, contractor, teacher, employee, agent, representative, and/or assessors or assigns from any and all manners of action or actions, lawsuits, debts, liens, contracts, agreements, promises, insinuations, liabilities, claims, rights, obligations, negligence, damages, loses, and expenses of any nature whatsoever, known and unknown for which I and/or we have may have or may ever hereafter against each and/or every of the above referenced persons and/or entities by reason of use of any of the facilities, participating in any course of exercise, and/or any affiliation whatsoever with the all above.
I and/or We represent that the student is in good physical and/or mental condition and that He/She has no impairments, ailments, disabilities or special circumstances to prevent them from undertaking any activity. And that if I and/or We have or become aware of a medical history, disorder, condition, concern, or any other doubt regarding any physical or mental issue that I and/or We should seek the consultation and/or examination of a Professional licensed physician before engaging in any and all activity or affiliation.
Furthermore, none of the parties above can unfortunately be held responsible, accountable, or liable for any and all personal property or public property in care of the undersigned. This includes but is not limited to damage, theft, or destruction of the same thereof and that I and/or We assume 100% full responsibility and risks of such property.
I and/or We finally acknowledge carefully reading, completely understanding, and agreeing 100% with the entire document releasing and discharging all and any future claims known and unknown expressly waiving any rights I may have against any of the above. I and/or We fully realize and understand that this is a binding legal document which completely eliminates any claims and effects any rights I may have with any affiliation specifically with Cottage Dance Academy LLC, all above, successors and/or assigns.
