EFFECTIVE SEASON
This Privacy Waiver and Consent Form applies exclusively to the regular dance season and is effective, once signed and ending June 30, 2027. A new Privacy Waiver is required for each dance season unless otherwise stated by the Studio.

1. PURPOSE OF THIS AGREEMENT
This Privacy Waiver and Consent Form (“Agreement”) outlines how Studio Twenty-Five Dance Company (“Studio”) collects, uses, stores, and shares personal information related to enrolled dancers and their parent(s) or legal guardian(s).

2. INFORMATION COLLECTED
By signing this Agreement, you acknowledge that the Studio may collect, store, and maintain personal information including, but not limited to:
• Student name, date of birth, address, and contact information
• Parent/guardian names, contact details, and emergency contacts
• Medical information, allergies, or health considerations voluntarily provided
• Attendance records and participation history
• Photographs and video recordings taken during classes, rehearsals, performances, or events (unless opted out in writing)

3. USE OF INFORMATION
Collected information may be used for the following purposes:
• Ensuring student safety and emergency response
• Registration, enrollment, billing, and account management
• Class scheduling, staffing, and event planning
• Studio communications (email, phone, text, newsletters, and announcements)
• Marketing and promotional materials, including digital, print, and online platforms

4. DISCLOSURE OF INFORMATION
Studio Twenty-Five Dance Company does not sell or trade personal information. Information may be shared only under the following circumstances:
• When required by law or legal process
• With emergency responders or medical providers in the event of an emergency
• With third-party service providers necessary for Studio operations (e.g., registration platforms, payment processors), who are required to maintain confidentiality

5. MEDIA RELEASE CONSENT
Unless a written request to opt out is submitted, I grant permission for Studio Twenty-Five Dance Company to use photographs and video recordings of myself and/or my child for promotional, educational, and marketing purposes without compensation.
Media Opt-Out:
A written opt-out request must be submitted to the Studio office. Opt-out requests apply only to future use and do not affect previously published materials.

6. DATA PROTECTION & SECURITY
The Studio takes reasonable administrative and technical measures to protect personal information. However, no data system is completely secure. By signing this Agreement, I acknowledge this risk and release the Studio from liability for damages resulting from an unauthorized data breach, to the extent permitted by law.

7. RIGHT TO REVOKE CONSENT
Consent may be revoked at any time by submitting a written request to the Studio. Revocation does not apply retroactively to information already collected, used, or shared in accordance with this Agreement prior to receipt of the written notice.

8. DURATION OF CONSENT
This Agreement remains in effect for the duration of the current regular dance season unless revoked in writing. A new Privacy Waiver must be completed for each subsequent season.

9. GOVERNING LAW
This Agreement shall be governed by and interpreted in accordance with the laws of the Commonwealth of Virginia.

ACKNOWLEDGMENT & CONSENT
SEASON TRANSITION & TEMPORARY COVERAGE
If the Participant continues to attend Studio Twenty-Five Dance Company classes, rehearsals, performances, or Studio-approved activities after the end of the regular dance season and prior to execution of a new season agreement, this Agreement shall remain temporarily in effect for a period not to exceed thirty (30) days solely for the purpose of covering such continued participation. Participation beyond this transition period requires execution of the current season’s agreement. By signing below, I acknowledge that I have read and understand this Privacy Waiver and Consent Form. I voluntarily consent to the collection, use, and disclosure of personal information as outlined above for myself and/or my minor child.
