STUDIO TWENTY-FIVE DANCE COMPANY PRIVACY WAIVER AND CONSENT FORM
1. Purpose of This Waiver
This Privacy Waiver and Consent Form ("Agreement") outlines how Studio Twenty-Five Dance Company ("Studio") collects, uses, stores, and shares personal information, including names, addresses, emails, medical history, photos, and other identifying details for both participants and guardians.
2. Information Collected
By signing this waiver, you acknowledge that the Studio may collect and maintain records that include but are not limited to:
Student name, date of birth, address, and contact details
Parent/guardian contact and emergency information
Medical and allergy details
Attendance records and participation history
Photos and video recordings for promotional use (unless opted out in writing)
3. Use of Information
Collected information may be used for the following purposes:
Emergency contact and safety
Registration, billing, and account management
Class and event planning
Studio communications (email, phone, text)
Marketing and promotional content (photos/videos)
4. Disclosure of Information
The Studio will not sell, trade, or otherwise transfer your personal information to outside parties except:
When required by law
To emergency personnel or medical providers in case of an emergency
To third-party service providers for studio operations (e.g., payment processing)
5. Consent to Media Release
Unless a written request is submitted to opt-out, I give permission for Studio Twenty-Five Dance Company to use photographs and video footage of my child(ren) or myself for marketing, promotional, and studio use in digital, print, and online media.
6. Data Protection and Security
Studio Twenty-Five Dance Company takes reasonable measures to safeguard personal data. However, no system is 100% secure. By signing this form, you acknowledge this risk and release the Studio from liability in the unlikely event of data breach.
7. Right to Revoke Consent
You have the right to revoke this consent in writing at any time. However, this does not apply retroactively to information already used or shared in accordance with this waiver.
8. Duration of Consent
This waiver remains in effect for the duration of your or your child’s enrollment in Studio programs unless revoked in writing.
