Registration Waiver

I hereby waive and absolve Eminence Dance Company and all divisions thereof of any and all liability and responsibility for injuries, sickness, accidents and/or acts of God incurred during participation in and/or instruction of camps, clinics, private coaching, choreography, and/or other dance, tumbling, or cheer related activity by myself or my child, whose name is listed above. In consideration of my signed release allowing my child to participate in an Eminence Dance Company’s activity, I am intending to be legally bound, do hereby, my heirs, executer, and administration, waive, release, and forever discharge any and all rights and claims for damage which I may have or which may hereafter accrue to me against Eminence Dance Company or respective teachers or employees, for any and all damages which may be sustained or suffered by me or my children in connection with Eminence Dance Company. In the event of injury/accident or sickness, Eminence Dance Company officials and instructors should contact the designated adult on this form. I hereby give my permission for my child to be photographed, videotaped and /or audiotape during any Eminence Dance Company activity. I further give permission for such photographs to be used in print or broadcast media as deemed appropriate for promotion of Eminence Dance Company activity or for publicity and promotion.
LATE FEES AND COLLECTIONS
Eminence Dance Company’s tuition due date is the 5TH day of every month. If full payment has not been received by this day, your account will be charged a $15 late fee. After 90 days, your account may be sent to a collection agency, where full payment will be required. If you are still delinquent after this said 90 days, your child/children will no longer be allowed to attend their classes until full payment has been received. Also, a new tuition contract will need to be put into place. Your child/children may also be subject to a new registration and its fees. In addition to tuition music, costume, and competition fees will be charged to any competing dancer. All of those fees are due at least 30 days prior to the first competition. If these fees are not paid, your account may be sent to a collection agency. If your account does go into collections, or small claims court you will be responsible for all the additional fees involved in addition to your account balance and late fees.
I hereby understand and agree to the Eminence Dance Company’s tuition and payment contract. I understand that I am fully responsible for payment, in full, on the 1st day of every month. I also agree to be charged additional fees if I do not pay tuition the 5TH day of every month. I also understand that if I do not follow this contract in full, after 90 days of missed payments my child/children will be dismissed from Eminence Dance Company until payment requirements have been fulfilled.
