All-Star Cheer & Dance Liability Waiver
Participation in all-star cheerleading, dance, tumbling, stunting, gymnastics, conditioning, choreography, competitions, performances, camps, travel, and related activities involves inherent risks of injury. These risks may include, but are not limited to, falls, collisions, physical contact with other participants, equipment failure, sprains, fractures, concussions, paralysis, illness, and other serious injuries.
Participants voluntarily assume all risks associated with participation in these activities.
The participant (and parent/legal guardian if under 18) acknowledges that they are physically able to participate in cheer and dance activities and understand the physical demands involved.
The participant (and parent/legal guardian) agrees to release and hold harmless the gym, program, organization, owners, coaches, employees, volunteers, staff members, and affiliated facilities from any and all liability, claims, demands, damages, injuries, or causes of action arising from participation in any cheer or dance-related activity, including travel to and from events, practices, and competitions.
The participant authorizes emergency medical treatment if necessary and understands that all medical expenses are the responsibility of the participant or parent/guardian.
Participants are expected to follow all safety rules, coach instructions, and standards of conduct. Unsportsmanlike behavior, bullying, harassment, or unsafe actions may result in removal from activities. No running in the gym, no tumbling outside of practice. If child is not enrolled in program and get hurt is is not on State of the Arts.
The organization may use photographs or videos taken during practices, events, or competitions for promotional, advertising, social media, or training purposes unless otherwise requested in writing.
