Acknowledgment of Risk

I, the undersigned, acknowledge and understand that cheerleading and its related activities (including, but not limited to, stunting, tumbling, jumps, and other physical exercises) involve physical exertion and carry with them inherent risks, including the possibility of personal injury, disability, or death. I acknowledge that these risks are a normal part of cheerleading, and I voluntarily assume all risks involved with participation in SOTA Cheerleading.

Assumption of Liability

I, the undersigned, hereby release, waive, and discharge the School of the Arts (SOTA), its officers, directors, coaches, staff, and volunteers (collectively referred to as “the Releasees”) from any and all liability, claims, demands, actions, or causes of action that I or my minor child may have arising out of participation in cheerleading activities, whether caused by the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

Medical Treatment

In the event of an injury, illness, or emergency situation, I consent to medical treatment being provided to the participant if necessary. I agree to bear all costs associated with any medical treatment and understand that it is my responsibility to ensure that the participant has medical insurance coverage.

Indemnity Agreement

I further agree to indemnify and hold harmless the Releasees from any and all claims, losses, or liabilities arising from any injury or damage resulting from my or my minor child’s participation in SOTA Cheerleading activities, including but not limited to claims brought by other participants, spectators, or third parties.

Acknowledgment of Understanding

I acknowledge that I have read and fully understand the terms of this waiver. I have had an opportunity to ask questions, and all of my questions have been answered to my satisfaction. I agree to follow all rules and guidelines established by SOTA Cheerleading, including those regarding safety, conduct, and participation.

Participant Consent
By signing below, I consent to the above waiver and acknowledge that I am voluntarily assuming the risks associated with cheerleading activities.
