Purely Dance LLC Terms and Conditions

I, on behalf of myself, my family and our respective heirs, executors, administrators, and assigns, hereby acknowledge and agree to the following terms and conditions regarding participation in dance classes, activities, events, and performances facilitated by Purely Dance LLC. I also agree to abide by any rules, regulations and policies set forth by Purely Dance LLC.
Assumption of Risk: I understand that dance activities involve physical exertion and potential risks, including but not limited to, personal injury, illness, or damage to personal property. I accept all risks associated with participation in Purely Dance LLC related activities.
Personal Property Release: I understand and agree that it is my sole responsibility to safeguard my personal property while attending or participating in any classes, events, or workshops. I hereby release Purely Dance LLC, its owners, instructors, employees, and representatives from all liability for loss or damage to my personal property while attending or participating in dance classes, activities, events, and performances.
Release of Liability: I hereby waive, release, and forever discharge Purely Dance LLC, its owners, instructors, employees, and representatives from any and all liabilities, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness that may be sustained while participating in dance activities.
Medical Emergency Release: In case of physical injury or medical emergency, I hereby authorize Purely Dance LLC to make necessary arrangements for transport to a medical treatment facility as necessary. All such transportation and medical treatment will be at my sole cost and expense. If the individual is under 18 years old or incapable of making a decision, I understand that Purely Dance LLC will attempt to notify the person(s) named as emergency contact(s).
Photo and Video Release: I grant Purely Dance LLC and its authorized representatives the irrevocable right to use photographs, videos, and audio recordings of us taken during dance classes, activities, events, and performances for promotional, advertising, and educational purposes, including but not limited to the studio's website, social media, brochures, newsletters, and other marketing materials.
I understand that I will not be entitled to any compensation, royalties, or any other consideration for the use of our image or likeness. I further waive any right to inspect or approve the finished product, including written copy that may be created in connection with the photographs, videos, or audio recordings.
I acknowledge and understand that Purely Dance LLC instructors, employees or representatives may prohibit the taking of photos and videos. I agree that I will honor any such restrictions imposed
I acknowledge and represent that I have fully informed myself of the content of the Purely Dance LLC Terms and Conditions by reading it before accepting it, and I understand that I do so of my own free act and deed. No oral representations, statements, or inducements, apart from the written statement, have been made. I further state that I am at least 18 years of age and fully competent to sign this agreement; and that I execute this release for full, adequate, and complete consideration fully intending to be bound by the same. I further state that there are no health-related reasons or problems, which preclude or restrict our participation in these activities, and that I will pay any medical costs that may be attendant as a result of injury.
