ASSUMPTION OF RISK
I/we realize that participation in dance classes and activities could result in some possible personal injury. Despite precautions being taken by JJ Dance Arts Instructors, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by JJ Dance Arts Instructors at JJ Dance Arts. I/we agree to release from responsibility JJ Dance Arts Instructors and JJ Dance Arts, facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold any JJ Dance Arts Instructor and JJ Dance Arts, liable for any personal injury including: scrapes, bruises, cuts, sprains, fractures, broken bones, concussions or any personal property damage/loss, which may occur on the premises before, during or after classes. I authorize JJ Dance Arts to take all reasonable steps to respond to medical emergencies and to seek medical treatment in the case of an injury.
LIABILITY RELEASE
I am the Parent / Legal Guardian of the dancer mentioned above
I fully understand and acknowledge that,
(a) risks and dangers exist while participating in physical activities such as dancing.
(b) the participation in such activities and/or use of such equipment may result in injury or illness including, but not limited to bodily injury.
I/we agree to release, waive, discharge, and hold harmless JJ Dance Arts, it’s owners,
and it’s employees from any liability, claims, actions or losses for bodily injury, property damage, or
otherwise which may arise out of The Minor’s participation in services provided by JJ Dance Arts. I also authorize JJ Dance Arts to take all reasonable steps to respond to medical emergencies and to seek medical treatment in the case of an injury. I/we agree to release from responsibility JJ Dance Arts Instructors and JJ Dance Arts, facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold any JJ Dance Arts Instructor and JJ Dance Arts, liable for any personal injury including: scrapes, bruises, cuts, sprains, fractures, broken bones, concussions or any personal property damage/loss, which may occur on the premises before, during or after classes.
PAYMENT POLICY
Any cancellations must be communicated via email to jj@dancearts.ca at least 1 month prior to the first day of summer camp and/or recreational season classes. For cancellations communicated up to 1 month before summer camp and/or recreational season classes begin, your payment is 50% refundable. The $50 Registration fee for any of our programs/classes will de deducted from your tuition and is non-refundable.
THERE ARE NO REFUNDS for cancellations that are not communicated via email to jj@dancearts.ca any later than 1 month prior to the start of summer camp and/or recreational season classes.
I agree to provide payment as outlined in the Fee Schedule by auto-payment via a valid credit card on file in the online portal. If the above is NOT provided, NSF charges will be posted to my account accordingly and interest will be charged at the rate indicated on my statements. If payment is not made on a timely basis, my student will be refused entry into class. I agree to provide one month's notice/one month's payment if my student wishes to withdraw.
MISSED CLASSES & ATTENDANCE
Tuition is calculated on a prorated basis and divided evenly across 10 monthly payments for the season, regardless of the number of classes in a given month. Please note that missed classes are non-refundable and cannot be credited or transferred. Consistency is key to progress, and we encourage regular attendance to support your dancer’s development.
PHOTOGRAPHY & VIDEO RELEASE
I give permission for my son/daughter’s class photographs and/or videos to be used for online publicity and advertising for JJ Dance Arts and Melissa JJ Moore.
