Diversity Performing Arts Center (DPAC)
Liability & Photography Waiver
By registering my child(ren) for classes at Diversity Performing Arts Center (“DPAC”), I acknowledge and agree to the following:
1. Assumption of Risk & Liability Waiver
I understand that participation in dance, movement, and related activities involves physical exertion and carries inherent risks, including but not limited to slips, falls, sprains, and other injuries. I voluntarily assume all risks associated with participation.
I release and hold harmless DPAC, its directors, staff, instructors, and affiliates from any liability for injuries, accidents, damages, or losses that may occur during participation in classes, rehearsals, performances, or related activities, whether on or off premises.
2. Medical Authorization
In the event of injury or medical emergency, I authorize DPAC staff to administer first aid and/or obtain emergency medical treatment for my child. I understand that I am responsible for all medical costs incurred.
3. Financial Responsibility
I understand that registration fees, tuition, and any other payments are non-refundable and must be paid in full according to DPAC’s policies. Missed classes do not result in credits or refunds unless otherwise stated in writing by DPAC.
4. Photography & Media Release
I grant DPAC full permission to photograph, video record, and/or use my child’s likeness in promotional materials, social media, advertising, and other lawful purposes related to DPAC programming. I understand these images/videos will not be sold to outside parties.
5. Code of Conduct
I agree that myself and my child(ren) will respect DPAC staff, students, and facilities. DPAC reserves the right to suspend or remove any student/family that violates policies or disrupts the learning environment.
By completing registration, I acknowledge that I have read, understood, and agree to the above terms.
