Media Release Waiver

1. Grant of Permission
I, the undersigned parent or legal guardian of the above-named child, hereby grant permission to Crestview Studios and its affiliates, employees, agents, and representatives (collectively referred to as "Crestview Studios") to use photographs, video recordings, audio recordings, and other media (collectively referred to as “Media”) of my child, either alone or with others, taken during participation in the Crestview NEXTGEN Dance Program, for appropriate promotional, educational, and advertising purposes.
2. Scope of Use
I understand that the Media may be used in a variety of formats, including but not limited to:
Social media platforms (e.g., Facebook, Instagram, YouTube)
Website(s) and digital platforms
Printed materials (brochures, flyers, posters)
Video productions, visuals, and commercials
News outlets, press releases, and promotional campaigns
Other promotional activities related to the Crestview Nextgen Dance Program or Crestview Studios
3. No Compensation
I acknowledge that no compensation will be provided for the use of the Media, and I waive any right to inspect or approve the finished product or other materials in which the Media may appear.
4. Release of Liability
I, on behalf of myself and my child, hereby release and hold harmless Crestview Studios, its officers, employees, agents, and representatives, from any and all liability, claims, damages, or losses that may arise out of or in connection with the use of the Media, including but not limited to any claims for defamation, invasion of privacy, or infringement of rights of publicity.
5. Duration of Use
This release shall remain in effect indefinitely unless revoked in writing by me. If I choose to revoke this consent, I understand that the Media will not be used in any new material after the date of the revocation, but may continue to be used in any previously existing materials.
6. Acknowledgment of Understanding
By signing below, I acknowledge that I have read and fully understand this Media Release Waiver. I agree to the terms and conditions outlined in this document, and I grant permission for my child’s participation in the Media usage as described above.
