Glow Athletics – Summer Camp Transportation Authorization

I hereby authorize Glow Athletics, including its staff, representatives, and authorized transportation providers, to transport my child to and from all Glow Athletics Summer Camp field trips and off-site activities.

This includes, but is not limited to:

Transportation to field trip destinations

Travel between camp-related locations

Emergency transportation if needed during camp hours

I understand that all transportation will be conducted by authorized personnel and that appropriate supervision and safety measures will be in place.

I release and hold harmless Glow Athletics, its staff, volunteers, and transportation partners from any and all liability that may arise from transportation-related incidents while my child is in their care during summer camp activities.
