Glow Athletics – Photo & Media Release Form

I hereby grant permission to Glow Athletics, its staff, representatives, and affiliated media, to photograph, film, and/or record my child during participation in any Glow Athletics camps, classes, or related activities.

I understand and agree that these images, videos, and recordings may be used by Glow Athletics for marketing, promotional, educational, or informational purposes, including but not limited to social media, websites, brochures, newsletters, and other official materials.

I acknowledge that these materials may be used without additional notice, review, or compensation. I release and hold harmless Glow Athletics from any claims arising from the use of such media.

This authorization is granted in perpetuity and applies to all Glow Athletics programs my child participates in.
