GLOW ATHLETICS PARTICIPATION WAIVER AND RELEASE OF LIABILITY
Program Name: Glow Athletics Programs
Program Dates: 2025-2026
By electronically signing this Waiver, I acknowledge and agree to the following:
1. Assumption of Risk
I understand and acknowledge that participation in Glow Athletics programs, including but not limited to fitness, training, or any other athletic activity (“Glow Athletics”), involves inherent risks, including but not limited to physical injury, falls, equipment malfunction, and accidents. I am voluntarily participating in the Program with knowledge of these risks and accept full responsibility for my participation.
2. Release of Liability
I hereby release, waive, discharge, and covenant not to sue Glow Athletics, its employees, officers, agents, volunteers, and affiliates (collectively, “Releasees”) from any and all liability, claims, demands, or causes of action that may arise out of or relate to my participation in the Program, including but not limited to injuries, damages, or losses sustained while participating in the Program, whether caused by the negligence of the Releasees or otherwise.
3. Medical Conditions
I certify that I am physically capable of participating in the Program and that I do not suffer from any medical condition that would impair my ability to safely participate. I agree to immediately notify a staff member of any medical condition, injury, or discomfort that may arise during my participation. I acknowledge that it is my responsibility to consult with a medical professional prior to participation if I have concerns about my health or ability to participate.
4. Indemnification
I agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or cost, including reasonable attorney’s fees, arising out of my participation in the Program, whether caused by the negligence of the Releasees or otherwise.
5. Photographs and Videos
I consent to the recording, use, and publication of any photographs or videos taken during the Program for promotional purposes by Glow Athletics, including but not limited to websites, social media, and marketing materials, without compensation to me.
6. Code of Conduct and Program Rules
I agree to follow all rules, regulations, and instructions provided by Glow Athletics and its staff. I understand that failure to comply with these rules may result in my removal from the Program without refund.
7. Acknowledgment of Understanding
I have read and fully understand this Waiver and Release of Liability. I am aware that by electronically signing this document, I am waiving certain legal rights, including the right to sue. I am signing this Waiver voluntarily and of my own free will.
By signing electronically, I acknowledge that I have read, understand, and agree to the terms outlined in this Waiver and Release of Liability.
