I, (Parent Name), the legal guardian of (Gymnast Name), give permission for Champion Mindset Gymnastics Inc. to take and use photographs and videos of my child. The photos and videos can be used for any legal use including but not limited to publicity, copyright purposes, illustrations, advertising, web content, and social media platforms to include the following: Instagram, YouTube, Facebook, and TikTok.
Furthermore, I understand that no royalty, fee, or compensation shall become payable to me by reason of such use.
