Lava City Cirque Privacy, Emergency Information & Photo/Video Consent

Personal Information
Lava City Cirque collects information necessary to register participants, administer classes and programs, process payments, communicate with families, maintain emergency contact information, and help instructors provide a safe training environment.

Participant information may include names, contact information, emergency contacts, age or date of birth, and relevant health, allergy, injury, or other safety information voluntarily provided by the participant or parent/guardian.

LCC will use this information for legitimate studio operations and will take reasonable measures to protect personal information.

Information may be shared with service providers used to operate the studio, such as registration or payment-processing providers, or when reasonably necessary for safety, emergency response, insurance, or legal requirements.

LCC does not sell participant personal information.


Emergency Medical Authorization
If a participant becomes injured or experiences a medical emergency and a parent, guardian, or emergency contact cannot be reached promptly, I authorize Lava City Cirque staff to contact emergency medical services and obtain reasonably necessary emergency assistance for the participant.

I understand that I am responsible for costs associated with medical treatment or emergency transportation.

I understand that LCC staff are not medical professionals and that this authorization does not require LCC staff to diagnose or provide medical treatment beyond their training.

I agree to keep the participant’s emergency contact and relevant safety information current.


Photo & Video Permission
Lava City Cirque occasionally photographs or records classes, camps, performances, events, and other studio activities for use on our website, social media, advertising, and other promotional materials.

If you do not want your child or participant photographed or recorded for promotional purposes, please select the Photo Opt Out option in the student portal after registration.

We will not intentionally use an identifiable image or video of a participant who has opted out.

Photo preferences may be changed at any time through the student portal.
