Liability Release Form

I understand every effort will be made to keep my child safe from injury at Studio 3:4, LLC. I also understand every effort will be made to contact me in case of an emergency. I recognize that there are certain inherent risks associated with the activities my child will be participating in at Studio 3:4, LLC when being instructed and taught dance and tumbling material during classes. I assume full responsibility for injury of my child or to myself (if applicable) and release further release and discharge Studio 3:4 for injury, loss or damage arising out of my family’s use of presence upon the facility of Studio 3:4. I hereby release Studio 3:4, LLC from any responsibility of dance and tumbling curriculum being taught to my child during classes. In case of accident, injury or COVID 19 exposure, I will not hold Studio 3:4, LLC or staff members liable.
