PHOTO & VIDEO RELEASE

I authorize, for myself and/or on behalf of a minor for whom I am legally responsible; Natalie’s School of Champions “NSC” and Natalie Watt to use the following information:

1) My and/or my child’s picture – in the form of photographic, motion picture, and electronic video images and recordings (Example: Recital DVD, Program Picture etc.)
2) My and/or my child’s voice – in the form of sound and video recordings (Example: Senior Farwell's)
3) My and/or my child’s name - in writing and printed materials. (Example: Recital Program)

I grant Natalie’s School of Champions, its subsidiaries, licensees, successors, and assigns the right to use, publish and reproduce, for all purposes, the information described in sections 1 through 3 above. This reproduction may occur in any media including, without limitation, cable and broadcast television, print, newspapers, magazine or the internet. It may be for the purposes of exhibition, distribution, promotion, advertising, sale, press conferences, meetings, hears or educational conferences. This grant of permission extends to all languages, media, formats and markets, including those devised in the future. This permission will continue forever unless I revoke it in writing.

I further grant Natalie’s School of Champions, all right, title, and interest that I may have in any of the information described in sections 1 through 3 above, in whatever medium that information might be published. I grant Natalie’s School of Champions the right to give, sell, transfer, and exhibit the information. I waive the right to receive payment for Natalie’s School of Champions' use of the information described in sections 1 through 3 above. I also waive the right to inspect or approve of (1) the information described in sections 1 through 3 above and (2) the manner of its distribution.

This release is provided on behalf of a minor and I certify that I am the parent or guardian of said child who is under the age of eighteen years, to whom this release applies and that I have the legal authority to execute this release.
