I, [Dancer’s Full Name] ___________________________________, hereby acknowledge and agree to the
following terms and conditions while participating in dance classes, workshops, and events at Ballet Folklorico
Sol de San Antonio (BFSDSA).
-Assumption of Risk:
I understand that dance activities involve physical exertion and the risk of injury. I voluntarily and
knowingly assume all risks associated with my participation in dance classes, including but not
limited to sprains, strains, fractures, falls, and other physical injuries.
-Health and Fitness:
I attest that I am physically fit and have no known medical conditions that would prevent me from
participating in dance classes. If I have any concerns about my health or physical ability to
participate, I will consult a medical professional before taking part.
-Release of Liability:
I hereby release, waive, discharge, and covenant not to sue Ballet Folklorico Sol de San Antonio
(BFSDSA), its directors, instructors, staff, volunteers, and agents from any liability, claims,
demands, actions, or rights of action arising out of any injuries, losses, or damages I may sustain
while participating in dance classes or related activities at the dance studio. This release includes,
without limitation, any injuries or damage that may result from the negligence of the dance studio
or its representatives.
-Ballet Folklorico Sol de San Antonio (BFSDSA) does not carry medical insurance for its
students. It is required that all dance students be covered by their own family’s insurance
policies, and if injury or sickness occurs, it is understood that the student’s own policy is
the only source of reimbursement.
-Upon registration, you and your family hereby authorize Ballet Folklorico Sol de San
Antonio (BFSDSA) to make necessary arrangements to transport my child or me to a
medical treatment facility as necessary in the event of a medical emergency. All such
transportation and medical treatment will be at the sole cost and expense of you and your
family.
-Indemnification:
I agree to indemnify and hold harmless Ballet Folklorico Sol de San Antonio (BFSDSA), its
directors, instructors, staff, volunteers, and agents from any claims, actions, suits, costs, expenses,
damages, or liabilities, including attorney fees, arising out of or in connection with my
participation in dance classes or related activities.
-Parent/Guardian Consent (If Applicable):
For participants under the age of 18, I, as the parent or legal guardian, consent to the terms of this
waiver on behalf of my child or ward. I also acknowledge that I am responsible for ensuring that
my child or ward follows all rules and instructions provided by the dance studio.
By signing below, I acknowledge that I have carefully read and fully understood all the provisions of the Ballet
Folklorico Sol de San Antonio (BFSDSA) Liability Waiver, and I voluntarily agree to be bound by its terms.
