A Time to Dance Performing Arts Studio
Liability Waiver, Medical Release & Media Consent

I, the undersigned participant and/or parent/legal guardian of the participant, understand and acknowledge that participation in dance classes, rehearsals, performances, camps, competitions, workshops, and related activities at A Time to Dance Performing Arts Studio involves physical activity and carries inherent risks, including but not limited to falls, collisions, muscle strains, sprains, broken bones, illness, or other serious injury.

I voluntarily choose to participate, or allow my child to participate, in activities offered by A Time to Dance Performing Arts Studio (“ATTD”) and assume all risks associated with participation, whether known or unknown.

I hereby release, waive, discharge, and hold harmless A Time to Dance Performing Arts Studio, including its owners, instructors, staff members, volunteers, choreographers, assistants, officers, directors, contractors, and any facilities used for studio activities, from any and all claims, liabilities, damages, injuries, losses, or causes of action arising out of or related to participation in studio activities, including transportation to and from events, except in cases of gross negligence or willful misconduct.

I understand that participation may require physical exertion and certify that the participant is physically capable of participating safely. I agree to disclose any medical conditions, allergies, injuries, physical limitations, or other health concerns that may affect participation. Any special medical or health concerns are listed on or attached to this form.

In the event of an emergency, I authorize A Time to Dance Performing Arts Studio and its representatives to obtain emergency medical treatment for the participant if I cannot be reached immediately. I understand and agree that I am solely responsible for all medical expenses, emergency transportation costs, and related treatment expenses incurred on behalf of the participant.

I understand that A Time to Dance Performing Arts Studio is not responsible for lost, stolen, or damaged personal property brought onto studio premises or to performances, competitions, camps, or events.

I agree to follow all studio rules, policies, safety procedures, and instructor directions. Failure to comply with studio policies or behavior expectations may result in dismissal from class or activities without refund.

Health & Illness Policy

To help maintain a safe environment for all students, families, and staff:

Students should not attend class if they are experiencing fever, vomiting, diarrhea, contagious illness symptoms, or other signs of sickness.
Students should remain home if they have been exposed to a contagious illness and are within a recommended isolation or recovery period.
Parents/guardians agree to monitor participants for symptoms of illness prior to attending classes or events.
ATTD reserves the right to refuse participation to any student showing visible signs of illness for the safety of others.

I understand that participation in public activities may increase exposure to contagious illnesses, including but not limited to COVID-19, influenza, RSV, and other communicable diseases. By participating in studio activities, I voluntarily assume all risks related to exposure to illness and agree not to hold A Time to Dance Performing Arts Studio liable for any illness, injury, or related complications resulting from exposure during participation.

Photo, Video & Media Release

I grant permission to A Time to Dance Performing Arts Studio to photograph, video record, or otherwise capture the participant’s image, voice, and likeness during classes, rehearsals, performances, competitions, camps, or studio events.

I understand these materials may be used by ATTD for promotional, advertising, educational, website, social media, recital, livestream, or marketing purposes without compensation or further notice.

Acknowledgment of Policies

I acknowledge that I have read, understand, and agree to abide by all studio policies, procedures, tuition/payment policies, recital guidelines, attendance expectations, and safety protocols established by A Time to Dance Performing Arts Studio.

I understand that policies may be updated periodically and agree to comply with all current studio guidelines.

I HAVE CAREFULLY READ THIS LIABILITY WAIVER, MEDICAL RELEASE, AND MEDIA CONSENT FORM AND FULLY UNDERSTAND ITS CONTENTS. I SIGN THIS DOCUMENT VOLUNTARILY AND ACKNOWLEDGE THAT IT IS LEGALLY BINDING.
