I hereby release Mary Ann Fortune's Dance Studio, Inc. instructors, and all event locations from any and all claims or liability due to personal injury or loss of property, which I (or my child) may sustain as a result of participating in any activity connected with Mary Ann Fortune's Dance Studio, Inc. I also give permission to use photos or videos from class instruction, recitals, exhibition performances or local talent and/or dance competitions of my child(ren) to be placed on www.maryannfortunesdancestudio.com.
