MASTER REGISTRATION AGREEMENT
By enrolling in classes, programs, or services at Ballet Etude Academy INC (“the Academy”), I, the student and/or parent or legal guardian of the student (“the Participant”), acknowledge and agree to the following terms and policies.
TUITION AND FEES
Tuition is due on the 1st of each month according to the payment schedule established by Ballet Etude Academy, unless the Participant elects to pay by semester or by full year.
A non-refundable registration fee is required at the time of enrollment.
All tuition, registration fees, performance fees, costume fees, and any other associated fees are non-refundable once paid.
NO REFUNDS
All tuition and fees paid to Ballet Etude Academy are non-refundable under any circumstances, including but not limited to withdrawal, absences, injury, illness, schedule changes, or dismissal from the Academy.
MAKE-UP CLASSES
While tuition and fees are non-refundable, Ballet Etude Academy may offer make-up class opportunities when available for classes missed due to illness, scheduling conflicts, or unforeseen circumstances. Make-up classes are not guaranteed.
WITHDRAWAL POLICY
If I choose to withdraw from Ballet Etude Academy, I understand that I remain responsible for all tuition and fees already paid and any outstanding balance on my account.
ACKNOWLEDGMENT OF RISK
I understand that participation in ballet training involves inherent risks, including the risk of personal injury. Despite all reasonable safety precautions taken by the Academy, injuries may occur.
With full knowledge of these risks, I voluntarily assume all risks of injury or damage associated with participation in ballet classes, rehearsals, performances, auditions, and related activities.
I agree to hold harmless Ballet Etude Academy INC, its owners, directors, instructors, staff, and affiliated facilities from any claims, demands, or causes of action arising from participation in Academy activities.
INJURY LIABILITY WAIVER
I hereby waive and release Ballet Etude Academy INC and any facilities housing its activities from any and all liability for injuries, sickness, or accidents incurred during participation in classes, rehearsals, performances, auditions, or related events.
I understand that in the event of injury, it is my responsibility and/or the responsibility of my insurance provider to seek, receive, and pay for all necessary medical treatment.
MEDIA RELEASE
I grant Ballet Etude Academy the right to photograph, video record, or otherwise capture my image and/or my child’s image during classes, rehearsals, performances, and Academy-related events.
I authorize the Academy to use such photographs and recordings, with or without names, for lawful purposes including but not limited to promotion, advertising, publications, social media, web content, and printed materials, without additional consent or compensation now or in the future.
PRIVACY AND DATA USE POLICY
By enrolling in Ballet Etude Academy, I consent to the collection and use of personal information including, but not limited to, names, addresses, phone numbers, email addresses, relevant medical information, enrollment records, and payment details.
This information is collected solely for the purpose of administering enrollment, processing payments, providing Academy services, and fulfilling related administrative responsibilities.
The Academy may use contact information to send program updates, announcements, and relevant communications. Clients may opt out of non-essential communications at any time by contacting the Academy.
Personal information will not be sold or shared with third parties except as required by law or as necessary for lawful debt collection, with appropriate safeguards in place.
All personal information is stored securely and accessed only by authorized personnel.
Requests to review, correct, or remove personal information may be submitted by contacting Ballet Etude Academy at 208-417-8593.
GENERAL CONDUCT
All students, parents, and staff are expected to maintain respectful and professional behavior at all times.
Disrespectful conduct, defamation, or bullying of any kind, including physical, emotional, verbal, or cyber behavior, will not be tolerated.
Violation of this policy may result in a meeting and possible dismissal at the discretion of the Artistic Director.
DROP-OFF AND PICK-UP
Ballet Etude Academy is not responsible for students before or after their scheduled class times.
Parents or guardians must ensure students are dropped off and picked up on time.
In the event of an emergency or delay, families must contact the Academy immediately at 208-417-8593.
PAYMENT ENFORCEMENT
Tuition and all associated fees must be paid in full and on time.
Late payments may result in a $10 late fee and/or exclusion from performances.
Continued non-payment may result in the account being sent to an external debt collection agency at the client’s expense.
CLASSROOM ETIQUETTE
Classes, including private lessons, may not be interrupted except in cases of emergency.
Parents and guardians should not enter classrooms or approach teachers or students during class time.
All communication should be directed through Ballet Etude Academy faculty or administration.
Classroom doors remain closed during class except during designated observation periods.
OBSERVATION POLICY
Observation Week occurs once per year and will be announced in advance.
During Observation Week, families may quietly observe class in person.
If in-person observation becomes distracting to a student, families may be asked to observe via video feed.
Parents will be provided access to classroom cameras through the WYZE app. Login details will be shared prior to each season.
FACILITY RULES
Students may not open windows or access restricted areas.
Studio cabinets and storage areas are off-limits unless directed by staff.
Running, roughhousing, or unsafe behavior is not permitted anywhere in the facility.
PROPERTY AND BELONGINGS
Ballet Etude Academy is not responsible for lost, stolen, or misplaced personal items.
Families are encouraged to leave valuables at home.
CHOREOGRAPHY AND INTELLECTUAL PROPERTY
All choreography, costuming designs, and Academy policies are the intellectual property of Ballet Etude Academy.
These materials may not be reproduced, shared, recorded, or distributed without written permission from the Artistic Director.
ADMINISTRATIVE NOTES
The Academy reserves the right to adjust class schedules, teachers, or timetables as necessary.

ACKNOWLEDGMENT AND AGREEMENT
By submitting my registration, I acknowledge that I have read, understood, and agree to all terms and policies contained in this Master Registration Agreement. I understand that enrollment and participation are contingent upon acceptance of these terms.
