Parent and Dancer Contract Agreement
I have carefully read, understand, and agree to the rules, standards, guidelines, policies and expectations set forth in the 2025-2026 SBS Dance Studio Competitive Dance Team Handbook, and I will adhere to all rules, standards, guidelines, policies and expectations outlined in this handbook and in the studio, as specified by SBSDS Administration and the Director.

I understand the financial responsibilities and understand that I am fully responsible for all of the SBS Dance Studio Competitive Dance Team expenses and fees. I understand that there are no refunds for any reason.

I understand SBS Dance Studio Competitive Dance Team requires a 100% commitment from my child, and I understand and agree to the consequences of failing to adhere to the SBS Dance Studio Competitive Dance Team Handbook and policies set forth by SBS Administration and Director. I agree to SBS Administration and Directors final decisions made throughout the season.

I fully understand, accept, and agree to abide by ALL terms, conditions, and obligations set forth by SBS Dance Studio Administration and the Director, SBS Dance Studio Competitive Handbook, and contract agreement and am giving my FULL YEAR (June 2025 -May 2026) commitment to SBSDS competitive dance team.

I also understand and agree that my child will be performing in the May 2025 Spring Showcase and understand that these guidelines and policies apply. I give my child fully permission to participate and will abide by these guidelines set forth by SBSDS.

Electronic Signature: I have had sufficient opportunity to read this entire handbook. I have read and understood it, and I agree to be bound by its terms. I understand that I am required to sign the 2025-2026 SBS Dance Studio Competitive Dance Team Handbook Contract Agreement form in the Parent Portal before my child is able to participate at SBS Dance Studio. By selecting the box, this means I accept all terms in this SBSDS 2025-2026 Handbook, including any other documents and/or amendments presented to you throughout year and this agreement form will stand as your electronic signature. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.
