Liability waiver

1) All students are to arrive on time, dressed in dance attire/practice uniform and have their hair neatly
pulled back off their fac. Keep up with your belongings in a dance bag.
2) The studio does not issue refunds, make up classes, or credits to students for classes missed due to
illness, vacation, religious observance, holidays or for any other reason.
3) Payments can be made by cash, credit, debit, PayPal, Cash app, Apple Pay or Zelle. We do not accept
personal checks or money orders.
4) Registration fees, uniform fees, and Tuition are non-refundable.
5) Uniform/competition/travel fees or any other studio fees are non-refundable.
6) I understand that dance and exercise classes involve physical movement and exertion and represent
that the student named is in good physical health and can perform the dances and exercises required
for participation in the classes. I waive all liability against Shining Diamonds Dance Studio and its staff
for any injury or illness as well as any loss of property sustained while participating in any classes or
activities of Shining Diamonds Dance Studio on or off the premises.
7) In the event that a parent or the emergency contact is not available, I give permission to the staff of
Shining Diamonds Dance Studio to obtain emergency medical treatment for any illness or injury that
may occur while the student is attending dance class.
8) Parents are not permitted to watch class. You must drop off and pick up on time (every 3rd late pickup
will result in a $25 late fee). Also, poor parent behavior (attitude, financial responsibility, timeliness of
attendance, unpreparedness for performances, slander, etc.) can result in expulsion of the student.
9) All classes are Drop-off classes. We need to have the waiting room made available to students coming
to and leaving from class, so we ask that you drop off your child and arrive in the parking lot 5 minutes
before the end of class. RESTROOMS ARE FOR STUDENTS AND STAFF ONLY.
10) The studio reserves the right to use photos and videos of students from dance class or
performances for advertising and marketing. Posts on social media of ANY routines must be approved
by director.
11) Payment and completion of this form verifies the acceptance of these conditions of enrollment
and the policies of Shining Diamonds Dance Studio.
12) Payments are due on the 1st of each month. After the 5th a $25 late fee is charged, and the
student is not permitted back to class until the fee and late fees are paid. There are no payment
arrangements or exceptions. This can result in your student not participating in shows, competitions,
and events so please pay on time.
